The role of neuroendoscopy in the evacuation of non-liquid chronic subdural hemorrhage

Chronic subdural hematomas are lifethreatening conditions whose incidence is increasing due to the frequent use of antiplatelet drugs. The “gold standard” of treatment is surgical relief by means of one or two borehole trepanations and temporary placement of a subdural drain.

The idea of multidisciplinary approach and increasing experience with endoscopic transnasal surgery and extended endoscopic approaches, such as transpterygoid, opened a wide range of indication of endoscopic surgery on skull base tumors. However, there are anatomical limitations of pure endonasal approach.

Unusually big colloid cysts. Differential diagnosis and strategy

Colloid cysts of the third ventricle typically manifest as regular round lesions situated between the foramens of Monroe, with diameters typically measuring a few millimeters. They are closely stuck to bilateral choroid plexuses of the frontal horns. Clinically, they may be incidental or, by obstructing cerebrospinal fluid (CSF) flow into the third ventricle, can lead to chronic or sudden intracranial hypertension.

Middle meningeal artery (MMA) embolisation has emerged in the last two decades as a possible way to treat chronic subdural hematomas (CSDH). The outer layer of the hematoma capsule recruits blood supply from the dural arteries which will contribute through the leaky capillaries of the membrane to the expansion of the hematoma.

Venous sinus stenosis is a part and a potential cause of a clinical syndrome called idiopath- ic intracranial hypotension (IIH). The venous stenosis may obstruct venous blood outflow, therefore result in increased intravenous pressure, reduced regional blood flow, intractable headaches, pulsatile tinnitus and pro- gressive visual loss.

The World Health Organization estimates that around 466 million people suffer from disabling hearing loss, with children constituting 7% of this population (2019). While cochlear implants aren’t suitable for all hearing-impaired individuals, sensory substitution principles, like non-invasive vibrotactile techniques, can be used to assist hearing-impaired patients.
