[Coumarins have been widely used for the prevention of arterial and venous thromboembolism. The importance of oral anticoagulants has steadily increased in internal medicine and cardiology in the past 20 years. Coumarins are vitamin K antagonists by inhibiting the synthesis of vitamin K-dependent coagulation factors in liver cells. Since the average half-life of hydroxycoumarin is longer than that of aceno-coumarin, the INR of patients treated with hydroxy-coumarin is more stable, thus making it especially suitable for long-term anticoagulant treatment. The main indications of coumarin therapy include secondary prophylaxis of venous thromboembolism, and primary and secondary prevention of arterial thromboembolism, particularly embolic stoke, in patients with cardiovascular disorders. This review summarizes the most important clinical issues of long-term oral anticoagulant therapy including indications, contraindications, dosing, therapeutic range, and laboratory monitoring. Special emphasis is placed on the role of hydroxy-coumarin in longterm oral anticoagulant therapy.]
[Although it has no cure, bronchial asthma can be efficiently treated. The use of inhaled corticosteroids and, more recently, long-acting β2- agonist aerosols have produced extraordinary improvement in clinical outcomes, particularly in relation to chronic breathing complaints. Today the aim of pharmacotherapy is to achieve a symptom-free state by the combined administration of long-acting β2-agonist and inhaled corticosteroids in the form of aerosol.]
[The incidence of coronary heart disease in women rises sharply in the years following menopause. Hormone replacement therapy involves the administration of oestrogen, which provides postmenopausal symptom relief and reverses the changes in calcium and lipid metabolism. Moreover, oestrogen is also postulated to engage multiple mechanisms that defend against hypertension. Early observations suggested that postmenopausal women treated with hormone replacement therapy have significantly reduced cardiovascular risk. However, the results of primary and secondary prevention randomized clinical trials confirmed an increased cardiovascular risk rather than a beneficial effect of hormone replacement therapy in highrisk women. Controversy between results of observational and randomized clinical trials may partly be due to the unexplored genetic background. The authors summarize the effects of oestrogen on lipids, inflammation, haemostatic parameters, blood pressure and vascular wall. Genetic factors that modulate the effect of oestrogen as well as current recommendations on hormone replacement therapy after menopause in high risk women are also presented.]
[INTRODUCTION - Natural antioxidants have been shown to indirectly act on signal transduction pathways and influence the induction of apoptotic genes in several ways. The black radish-based granules contain several bioactive components including isothiocyanates, flavonoids, vitamins and trace elements that can modify various molecular biological pathways. The aim of this study was to find out whether this preparation has an effect on the redox homeostasis of patients with inflammatory bowel disease. PATIENTS AND METHODS - Data from 32 middleaged patients (13 male, 19 female) with moderately active IBD were evaluated. Half of the patients (6 male, 10 female) received 0.2 g/day granules for 6 months in addition to regular treatment. Apart from other routine laboratory measurements, redox parameters including plasma reducing power, H-donor ability, free SH groups and IL-6, IL-1, TNFalpha, bile acids, cholesterol, triglycerides, red blood cell chemiluminescence and HbA1c level were determined. RESULTS - The black radish-based granules had no effect on the activity of inflammatory bowel disease. Beside a positive subjective judgement, bile acid levels had slightly increased and the values of redox parameters had decreased in the plasma by the end of the study period. The granules significantly diminished red blood cell chemiluminescence and moderately reduced the HbA1c-level. CONCLUSION - The results show that the granules rich in antioxidants and isothiocianate have a marked effect on the redox homeostasis of IBD patients even at low doses.]
[INTRODUCTION - The risk of eating disorders is higher among young women where a slim body ideal dominates. In contrast, the male body ideal is based on the athletic type. The aim of the present study was to assess the relevance of the athletic ideal as a risk factor in certain professional subcultures of young men with respect to intensive workout and body dissatisfaction. These can be important risk factors of muscle dysmorphia in certain populations. METHOD - Body and eating attitudes, as well as workout habits of 1232 military and “general” college male students were studied using questionnaires. The Eating Disorder Inventory was used for all subjects, and general demographic data and information on their bodybuilding practice were also obtained. RESULTS - General college students had a significantly higher mean body mass index than military students (23.9 vs. 23.5 kg/m2). The prevalence of those engaged in bodybuilding was surprisingly high among the military students (64.5%). Comparisons between and within groups also showed significantly different scores on several subscales of the Eating Disorder Inventory. CONCLUSION - The findings suggest that male military college students tend to have protective factors against the psychopathological features of eating disorders.]
