Intrasellar chordoma: A difficult preoperative diagnosis

Intrasellar chordomas are extremely rare tumours. To the best of our knowledge, only 41 cases werepublished in the literature. They are often confused with cranopharyngiomas and pituitary adenomas. Surgical decision varies a lot from one diagnosis to the other. Intrasellar chordoma have uniqueradiologic features that can help preoperative diagnosis. Management and prognosis of intrasellarchordoma are different from other intracranial chordomas. We share the case of a 77-year-old manwith a sellar and suprasellar lesion. Subtotal resection by transsphenoidal approach and radiationtherapy were performed. We describe imaging findings and remind the main lines of management andthe prognosis of intrasellar chordomas.
