Taming a lion: Multisession GKRS for complex AVM

From the beginning of neurosurgery, the very question of successful management of an intracranial arteriovenous malformation has been a challenging task. In pursuit of gaining control over this difficult neurosurgical ailment many treatment options have been proposed and practiced but none proofs to be a panacea for complex AVMs. Stereotactic radiosurgery (SRS) has proved to be a wonderful and effective alternative frequent option for AVM. In this chapter, authors discuss the current evidence, nuances and future of SRS in the management of AVM. Treatment of low grade AVM is non-controversial except for the ARUBA enthusiasts, however complex SM grade IV and V AVMs demand a judicious approach of intervention and the methodology to make a meaningful change in the disease management. Radiosurgery has proven especially effective in this pursuit with dose and volume fractionation in different regimens. It maintains an impressive balance between nidus obliteration and complication profile while ensuring a favourable change in the natural history.
