Intramedullary glioblastoma is an extremely rare entity accounting for only 1.5% of spinal tumors. It is characterized by rapid progression.
Here we report the case of a 51-year-old patient who presented with mixed low back pain associated with a urinary retention. The patient’s condition rapidly deteriorated, leading to paralysis and confinement to bed. Clinical examination revealed paraplegia with pyramidal syndrome in both lower limbs. Spinal MRI showed an intramedullary lesion at the level of D11-D12 and L1.the patient was operated with partial excision.Histopathologic examination reveals a high grade glioma.
The patient underwent adjuvant radio-chemotherapy, but remained paraplegic at two years postoperative follow-up, with no tumor progression or secondary localizations.
Glioblastoma of the conus medullaris is a rare entity. treatment is based on surgery followed by adjuvant treatment and the prognosis is unfavorable in the majority of cases leading to fatal outcomes.
