[The significance of interleuktin-6, it's molecular regulation in acute phase reaction]

FALUS András, BÍRÓ Judit, RÁKÁSZ Éva

[The multiple effectiveness of a hormone-like cytokine, interleukin-6 (IL-6) is highly important in a series of immunological, neuroendocrinological and inflammatory reactions of the mammalian organism. In this review a complex view of the gene and its regulation of IL-6 and the significance of the cytokine network in acute phase reaction is discussed. Manipulation of this complex network studied by the methodology of molecular biology and genetics provides entirely new approaches for the biotechnological and pharmacological regulation of inflammation.]

[Call for support for the Healthy Breasts Foundation]

MONOS Emil

[Hungary is one of the world's leading countries in the grim international statistics on cardiovascular disease mortality, with around 80 000 people dying every year from serious cardiovascular disorders. This figure represents nearly 55% of all deaths! The vast majority of heart disease is related to the failure of the heart's own coronary arteries, and a significant proportion of brain disease is related to damage to the brain vessels themselves.]

[The application of argon laser in cosmetological therapy]

DIMITRIJE Katalinic

[The author was one of the first in 1976 who began to use the various types of lasers for plastic-esthetical reasons and for therapy of skin lesions. He considers the indication spectrum of the argon laser to be the largest and its results are the best. The author summarizes and makes known the tissue-effects of the argon laser and compaes it with the tissue-destruction of other lasers and classical methods. The quality of the tissue destruction determines the indications and the applications, among them the therapy of the port wine syndrome. In the author's opini on, not the exposition time and the energy of the argon laser could be exactly determined. The experience of the physician plays the most important tole in the therapy. ]

[The mode of action of bath therapy]

BENDER Tamás

[Bath therapy includes both hydro- and balneotherapy. During hydrotherapy the patient is treated by the physical parameters of the water. The effect of balneotherapy is based on the penetration of the mineral water. Its me chanism is not yet clear. There are several double-blind studies which prove the benefits of balneotherapy. Further studies are needed to judge the objective effects of balneotherapy. ]

[Correlation between clinical data and histologycal findings assessing the severity of ulcerative colitis]

NAGY Ferenc, KARÁCSONY Gizella

[ A knowledge of the current activity of ulcerative colitis is essential in planning appropiate follow-up. Histological findings obtained from 209 colonoscopies were compared with the values of 22 clinical variables. One way analysis of variance was carried out. Of the 22 variables 11 proved to be significantly different between the groups of the clinically valuable classification (inactive, chronic, acute). Each of the three groups differed in the number of bloody stools. The acute inflammatory process was characterized by the high numbers of bowel movements, mucus and bloody mucus defecation, bowel movements at night, and haematocrit values suggesting anaemia. The serum iron levels in the inactive group differed significantly from the levels of both the acute and the chronic groups. The count of thrombocyte and the general well being were found to be valuable only in the differentiation between the acute and the inactive inflammatory processes. The computed haematocrit value proved to be useful in distinguishing the acute stage from the inactive and the chronic process. Out of the 22 variables only 11 had clinical importance in the evaluation of the activity of ulcerative colitis. ]

[First International Study of Infarct Survival]

MATOS Lajos

[In the treatment period (0-7 days), vascular mortality was significantly lower in the atenolol group (3.89%) than in controls (4.57%), representing a 15% reduction in mortality, but within very wide 95% confidence interval (1- 27%). Vascular mortality at one year was also found to be significantly lower (atenolol: 10.7%, control: 12.0%,2 p<0.01). After day 365, there was slightly more vascular mortality in the active agent-treated group, but this was not significant (2p = 0.07). The administration of a beta-blocker significantly increased the use of inotropic agents (5% in the atenolol group, 3.4% in the control, 2p < 0.001), especially in the first two days (days 0-1), but it was during this period that the improvement in vascular mortality was most pronounced (atenolol: 121, control: 171 deaths)]

[An estimation of the frequency of blepharospasm and hemifacial spam in hungary]

BIHARI József, TOMCSIK Miklós, BENYÓ Gábor

[Authors estimate the frequency of two focal dystonias, blepharospasm and hemifacial spasm in different general practices in Hungary. There are no similar data published in the literature. This epidemiologic study was an important step in the development of a center to treat focal dystonias in Hungary, since therapeutic results with botulinum toxin injections are excellent in these disorders. ]

[Second International Study of Infarct Survival]

MATOS Lajos

[The active agents already significantly reduced vascular mortality at the first study time point (week 5) (streptokinase vs. placebo: 9.2 vs. 12%, aspirin vs. placebo: 9.4 vs. 11.8%, 2p < 0.00001 for both, streptokinase and aspirin together vs. placebo: 8.0 vs. 13.2%). The combined administration of streptokinase and aspirin was significantly more effective than separately (2p < 0.0001). Active treatment also appeared to be beneficial when started relatively late after the onset of pain. The incidence of transfusion was 0.5% in the streptokinase group and 0.2% in the placebo group) and there were more cerebral haemorrhages (streptokinase: 0.1% compared with none in the placebo group). Aspirin alone significantly reduced the incidence of re-infarction leading to death (aspirin: 1%, placebo: 2%), non-fatal stroke (0.3% vs 0.6%) and did not lead to more bleeding requiring transfusion.]

[The level and structure of AIDS-related knowledge in hungary (1991)]

TÓTH László, KÁDÁR Erika, FORGÁCH István

[A survey was carried out with the purpose of collecting data on AIDS-related knowledge, attitudes and behavior. The results showed that the level of knowledge being at the foreground of conscious ness is rather low and increases as the questi ons tend to point toward each element of the knowledge. The highest level of knowledge can be found in the passive domain of knowledge. The structure of knowledge based on the correct answers shows an inconsistent picture in the examined domains of knowledge: there is a predominance of social-practical knowledge in the passive domain while in the active domain of knowledge the biomedical knowledge prevails. Comparing our data with those of previous researches in Hungary, a decrease was observed in the level of AIDS-related knowledge since 1987. There is a need to reconsider the whole longterm campaign against AIDS, on the one hand to create a longterm strategy instead of a shortterm one and on the other hand to force the individual into an active attitude instead of a paternalistic view. ]

[Psychoterapy as medical activity ]

SZŐNYI Gábor

[In the clinical practice we can use the medical model, the algorithm of examination, indication, choice of therapy, therapeutic process, outcome, control also in case of psychotherapies. As the location and the cause of the disorder, the place and the tool of the therapy are the same, namely the human psyche and the human interpersonal relationships the psychotherapies demand differing attitude from the somatic medicine at several points. The basic position should be summarized under the psychotherapeutic attitude. Main common parts of the psychotherapy methods are the therapeutic contract and the therapeutic relation. The different components influence the process through the person, the inner world of the patient and the therapist. Therapeutic interventions cannot be judged picked out of these interrelationships. ]

[Interdisciplinary quality assurance of hospital infrastructure Part II]

EUGEN Hauke

[The common task of ensuring the quality of patient care as a whole makes it clear that services not directly provided are not an end in themselves. They merely provide the conditions under which the medical-care and service process can take place at all, within the framework of the division of labour. The service requirements for indirect services therefore essentially come from doctors, nurses and those who are directly involved in the treatment of patients. ]

[Some things are becoming clearer, but other factors are still confused, despite, or perhaps because of, the arsenal of regulations. This situation is not helped much by the various meetings and briefings at which ministerial figures give answers to the questions posed in various ways that are understandable and interpretable - dodonai, one might say. In addition to the regulations, there are loopholes that can be exploited.]

LANTOS István

[I have a very kind young colleague who became a district doctor 4 years ago after nearly 10 years of hospital practice. She has since married and is currently at home with her two-year-old child. Due to changes in primary care, she has not lost her job but she lost her patients. Instead of her, other people who replaced her are collecting insurance cards from citizens in her former district. Her child is growing up, she is legally employed, only when she returns she will have no one to treat. ]
