[Europe’s highest COVID-19 mortality experienced behind the former iron curtain]

[While 11 out of the 15 first European countries with highest COVID-19 mortality rates are Post-Socialist countries (73%), and only 9 out of the 31 countries on place 16-46 (29%) are Post-Socialist. All of the top 17 European countries with the highest cumulative excess-mortality since the beginning of the COVID-19 pandemic era, are Post-Socialist countries (100%) as well as 19 out of the top 20 (95%). The dividing line between high and low COVID-19 mortality of European countries follows exactly the path of the former Iron Curtain. In addition, out of the 43 data reporting European countries, all the 18 with the lowest vaccination rate (100%) are Post-Socialist. The main cause of these differences in European COVID-19 mortality may be the traditionally disadvantageous physical and mental condition of the population of Post-Socialist countries, but this problem needs further, more intense investigations.]

[100 years of insulin]

[Insulin is 100 years old. Concerning this anniversary, this study reviews the main events before and after the insulin era, the wider range of insulin products and the chronology of main clinical innovations related to the insulin administration. It also presents the history of related devices as delivery pens, insulin pumps and automated treatment systems. It points out that, al­though insulin dosing is an external replacement for missing or insufficient endogenous hormone production as related to the actual demand, it is to this date the general treatment available for all types and stages of this condition.]

[Cardiovascular prevention 2021 – Guidelines of European Society of Cardiology 2021. Cardiovascular prevention at individual level]

[In August 2021, the European Society of Cardiology (ESC) published its practical guidelines for cardiovascular prevention. In Part 1 of our publication, we discussed cardiovascular risk factors, risk assessment, cardiovascular risk assessment in presumably healthy individuals of different ages, people with proven atherosclerotic cardiovascular disease, people with diabetes mellitus, and factors influencing risk. In Part 2, conditions that influence cardiovascular risk were detailed. In the current Part 3 of this publication, we review personalised cardiovascular prevention, non-pharmaceutical and pharmaceutical treatment of specific risk factors, emerging treatment options, and par­ticipation in cardiovascular rehabilita­tion and prevention programmes based on ESC guidelines.]

[Insulin: alfa and omega]

[Type 1 diabetes (T1DM) is caused by the autoimmune death of insulin-producing β-cells. In the background there is by all probability the primary antigen (if the theory of primary antigen is correct) the insulin itself or the proinsulin. However, treatment of T1DM is virtually equivalent to the insulin treatment. The earlier we recognize T1DM and start treatment, the better gly­caemic results we achieve, the more likely we can maintain all functional β-cell stocks. Insulin also plays a key role in the immunopathogenesis, development and treatment of T1DM, thus insulin is the alpha and omega in this condition.]

[Modern insulin therapy using insulin pump or insulin pump and sensor]

[Insulin was discovered 100 years ago. At this anniversary the present study concerns the applications of insulin pumps and sensors, emphasising the latest options in Hungary. Since January 1, 2020 the above devices are provided with 98% coverage by the National Health Insurance Fund for underage Type 1 diabetics and high school students up to 24 years. Adults with Type 1 diabetes may also obtain an insulin pump and sensor with 80% coverage of the so­cial health insurance. A sensor distributed by an independent local company will also be available in the upcoming months. It should be suggested that more and more unstable Type 1 diabetics use the insulin pump and sensors since this treatment stabilise the metabolism reduces the glucose level excesses and minimise the glucose varia­bility. All these measures are lowering the probability of diabetes related comp­lica­tions.]

[Focus on Lege Artis Medicinae (LAM)]

[Three decades ago, LAM was launched with the goal of providing scientific information about medicine and its frontiers. From the very beginning, LAM has also concerned a special subject area while connecting medicine with the world of art. In the palette of medical articles, it remained a special feature to this day. The analysis of the history of LAM to date was performed using internationally accepted publication guidelines and scientific databases as a pledge of objectivity. We examined the practice of LAM if it meets the main criteria, the professional expectations of our days, when publishing contents of the traditional printed edition and its electronic version. We explored the visibility of articles in the largest bibliographic and scientific metric databases, and reviewed the LAM's place among the Hun­ga­rian professional journals. Our results show that in recent years LAM has gained international reputation des­pite publishing in Hungarian spoken by a few people. This is due to articles with foreign co-authors as well as references to LAM in articles written exclusively by foreign researchers. The journal is of course full readable in the Hungarian bibliographic databases, and its popularity is among the leading ones. The great virtue of the journal is the wide spectrum of the authors' affiliation, with which they cover almost completely the Hungarian health care institutional sys­tem. The special feature of its columns is enhanced by the publication of writings on art, which may increase Hungarian and foreign interest like that of medical articles.]

[Mentalizing deficit among patients with traumatic brain injury ]

[Mentalization or theory of mind as an aspect of our social cognition, is our ability to infer mental states of others (intentions, desires, thoughts, emotions) and to predict their behavior accordingly. This function significantly affects our participation and orientation in the social world and plays an important role in conversational situations, social interactions, social integ­ration and adaptation. The brain regions that serve as the basis for mind-reading function can be damaged as a consequence of traumatic brain injury, which frequently occurs among the younger population. Traumatic brain injury can cause focal or diffuse cerebral injuries, often leading to theory of mind deficit. In this topic such publications were researched that compared theory of mind ability between traumatic brain injury patients and control subjects (comparative case-control studies). We searched for the studies in the following internet based/online databases: PubMed, Web of Science, ScienceDirect, Google Scholar, APA PsycNET (PsycARTICLES) and EBSCO Host. The search was performed using the following key word combinations: theory of mind or mentalizing or social cognition AND traumatic brain injury or head/brain injury or diffuse axonal injury. Based on the results of the included and processed studies (21 pc), traumatic brain injury often leads to mentalization deficit with different severity. With this present review we aim to draw attention to the fact that the appearance and severity of mind reading dysfunction can considerably affect the outcome of the disease, the length of rehabilitation time and the prognosis of traumatic brain injury patients. Besides this, with this review, we aim to take sides in whether theory of mind ability is domain-specific or domian-general based on studies including traumatic brain injury patients.]

[New, innovative prognosis calculator for patients with metastatic spinal tumors]

[The aim of our research was to create a scoring system that predicts prognosis and recommends therapeutic options for patients with metastatic spine tumor. Increasing oncological treatment opportunities and prolonged survival have led to a growing need to address clinical symptoms caused by meta­stases of the primary tumor. Spinal metastases can cause a significant reduction in quality of life due to the caused neurological deficits. A scoring system that predicts prognosis with sufficient accuracy could help us to achieve personalised treatment options. Methods – We performed a retrospective clinical research of data from patients over 18 years of age who underwent surgery due to symptomatic spinal metastasis at the National Institute of Mental Disorders, Neurology and Neurosurgery between 2008 and 2018. Data from 454 patients were analysed. Survival analysis (Kaplan-Meier, log-rank, Cox model) was performed, network science-based correlation analysis was used to select the proper prognostic factors of our scoring system, such that its C value (predictive ability index) was maximized. Multivariate Cox analysis resulted in the identification of 5 independent prognostic factors (primary tumour type, age, ambulatory status, internal organ metastases, serum protein level). Our system predicted with an average accuracy of 70.6% over the 10-year study period. Our large case series of surgical dataset of patients with symptomatic spinal metastasis was used to create a risk calculator system that can help in the choice of therapy. [The facets of creativity in the light of bipolar mood alterations]

[The link between creativity, as the highest expression form of human achievement, and bipolar disorder came into focus of scientific investigations and research. Accomplished writers, composers and visual artists show a substantially higher rate of affective disorders, prodominantly bipolar mood disorders, comparing to the general population. Then again, patients afflicted with bipolar II subtype (hypomania and depression), as well as persons presenting the mildest form of bipolar mood swings (cyclothymia) possess higher creative skills. It evokes therefore that certain forms and mood states of bipolar disorder, notably hypomania might convey cognitive, emotional/affective, and motivational benefits to creativity. The aim of this paper is to display expression forms of creativity (writing, visual art, scientific work) as well as productivity (literary and scientific work output, number of artworks and exhibitions, awards) in the light of clinically diagnosed mood states at an eminent creative individual, treated for bipolar II disorder. Analysing the affective states, we found a striking relation between hypomanic episodes and visual artistic creativity and achievement, as well as scientific performance, whereas mild-moderate depressed mood promoted literary work. Severe depression and mixed states were not associated with creative activities, and intriguingly, long-term stabilised euthymic mood, exempted from marked affective lability, is disadvantageous regarding creativity. It seems, thereby, that mood functions as a sluice of creativity. Nevertheless, it is likely that there is a complex interaction between bipolar mood disorder spectrum and psychological factors promoting creativity, influenced also by individual variability due to medication, comorbid conditions, and course of disorder.]
