[The role of steroid hormones in the protection against oxidative stress]


[The incidence of vascular diseases increases in postmenopausal women. This negative trend seems to be controllable favourably by hormone replacement therapy (HRT). Based on the free radical mediated mechanism of atherogenesis the authors try to find the reasons for these observations on the basis of their and others’ data. They consider the increase of activity and amount of neutrophil enzyme myeloperoxydase by certain steroid particulars one of the most important elements of the vasoprotective effect of HRT. This increase diminishes the production of superoxide anion by the inhibition of the NADPH-oxidase enzyme. The observation proved firstly by the authors, that the plasma levels of myeloperoxydase - and so the ability for the inhibition of superoxide anion production by neutrophils - decreases in elderly people, might explain the higher incidence of some free radical mediated illnesses in the elderly. The authors also give a short review of antioxidant effects of steroids in general.]

[The importance of vitrectomy in the management of diabetic retinopathy in proliferative stage]


[Vitrectomy is a closed microsurgical technique, by which intraocular tissue, blood and foreign bodies can be removed through a small incision in the pars plana with an automated suction-cutting device (vitrectom), while maintaining normal or slightly elevated intraocular pressure. This technique requires many accessory instruments (microscope, endoillumination, intraocular instruments, endolaser). The main application of vitrectomy is the treatment of severe complications in diabetes mellitus: diabetic retinopathy in the proliferative stage. Vitrectomy is indicated in diabetic retinopathy for the removal of intraocular bleeding and for the treatment of complications of diabetic proliferative vitreoretinopathy (recurring bleeding, tractional and rhegmatogenous retinal detachment). In the case of intraocular bleeding, early vitrectomy is better than delayed, providing good visus (V: 0,5) in 25% of all patients (based on literature data). In retinal detachment cases of tractional origin, surgery provides satisfactory visus for self-care in 2/3rd of all patients.]

[Gastrointestinal stromal tumor]


[Gastrointestinal stromal tumors were subject of much controversy in the last decades. This type of tumors was delineated from the leiomyoma - leiomyosarcoma group. GISTs show variable histological picture, moreover, they are capable of dual (neurogen and myogen) differentiation as it is proved by immunohistochemical and ultrastructural studies. These tumors have relatively good prognosis, only 10-30% of them is malignant, although it is difficult to predict their behaviour in a given case. The most reliable signs of malignancy - cytological pleoimorphism, high mitotic activity, proliferation index above 10 %, and aneuploid DNA content - are generally accepted. The treatment of choice is excision, but not enucleation. Radical surgery is not necessary, since lymph node metastasis is exceptionally rare. Further investigations revealed electronmicroscopical and immunohistochemical (CD 34, CD 117) similarities of tumor cells and interstitial cells of Cajal (ICC) located in the wall of the bowel. The results of these investigations led to the theory that cells of GIST and ICCs are of the same stem cell origin. Molecular genetic studies are also of great help in the differential diagnosis and in predicting the prognosis of GISTs. Mutations in the c-kit gene can not be detected in leiomyomas, so they are thougt to be specific for GISTs. Mutation of exon 11. of chromosome 4. is observed only in malignant GISTs.]

Cyanocobalamin and cholecalciferol synergistically improve functional and histopathological nerve healing in experimental rat model

Introduction - Peripheral nerve injury (PNI) is a frequent problem among young adults. Hopefully, regeneration can occur in PNI unlike central nervous system. If nerve cut is complete, gold standard treatment is surgery, but incomplete cuts have been tried to be treated by medicines. The aim of the study was to evaluate and compare clinical and histopathological outcomes of independent treatment of each of Vitamin B12 (B12) and Vitamin D3 (D3) and their combination on sciatic nerve injury in an experimental rat model. Materials and methods - Experimental animal study was performed after the approval of BEH Ethics Committee No. 2015/10. 32 rats were grouped into four (n=8) according to treatment procedures, such as Group 1 (controls with no treatment), Group 2 (intraperitoneal 1 mg/kg/day B12), Group 3 (oral 3500 IU/kg/week D3), Group 4 (intraperitoneal 1 mg/kg/day B12+ oral 3500 IU/kg/week D3). Sciatic Functional Index (SFI) and histopathological analysis were performed. Results - SFIs of Group 2, 3, 4 were statistically significantly higher than controls. Group 2 and 3 were statistically not different, however Group 4 was statistically significantly higher than others according to SFI. Axonal degeneration (AD) in all treatment groups were statistically significantly lower than in Group 1. AD in Group 4 was significantly lower than in Group 2 and 3; there was no significant difference between Group 2 and 3. There was no significant difference between Group 1,2 and 3 in Axonolysis (A). But A of Group 4 was significantly very much lower than all others. Oedema- inflammation (OE-I) in all treatment groups were significantly lower than in Group 1; there was no significant difference between Group 2 and group 4. OE-I in Group 2 and 4 were significantly lower than in Group 3. There were no significant differences between Group 1, 2 and 3 in damage level scores; score of Group 4 was significantly lower than of Group 1. Conclusions - B12 and D3 were found effective with no statistically significant difference. But combined use of B12 and D3 improve nerve healing synergistically. We recommend combined use of B12 and D3 after PNI as soon as possible.

[The healing environment: The landscape architecture’s aspects of the psychiatric institutional gardens development]

[According to researches, the surrounding environment of the people has a significant impact on the human well-being, its perception of welfare and may influence its health condition. Even the sight of a spacious, well-maintained, healthy densely vegetated area can strengthen the human body, so it may promote patients by the healing process. On the one hand, this study aims to overview the international literatures and best practises of the healingand therapeutic gardens. Our other goal was to show the potential feasibility of the theoretical principles by preparing analysis, evaluation and a detailed conception plan for a chosen institution garden. We also formulated generally applicable design guidelines for psychiatric home garden design. This research is aimed to contribute the creation an important objective of the European Union, to achieve equal opportunities.]

[Severe intracranial hypotension in an adolescent with Marfan syndrome. Case report]

[Spontaneous intracranial hypotension is a rare complication of connective tissue disorders. One of them is Marfan syndrome. It predisposes the patient to meningeal diverticula. Possibly after minor unrecognised head trauma or secondary to Valsalva manoeuvre cerebrospinal fluid leak from meningeal diverticula can happen. It causes postural headache. We describe a 15 year old adolescent female newly diagnosed with Marfan syndrome, who presented with intractable postural headache. Our patient's brain MRI showed bilateral frontal subdural effusion, narrow ventricles, downward displacement of the brainstem, enlarged cervical venous plexi, dural ectasias and wide nerve root sleeves. Radionuclide cisternography demonstrated CSF leaks at multiple sites. She could not be treated conservatively, but was successfully treated by epidural saline injections. Control brain and cervical MRI confirmed her healing, too. At the two and half year follow up visit, she was completely well. So we recommend this easiest technic to use before epidural autolog blood patches.]

[Physicians’ attitudes and suppositions in 2001 in the Polyclinic of the Hospitaller Brothers in Budapest]


[INTRODUCTION - Following the total socioeconomic changes in Hungary in 1989, no scientific analysis has been made about physicians' religious attributes in the new circumstances. These attributes, after four decades of political anticlericalism may affect essentially the ideological and socio-economic patterns of medical professionalism. Hungary’s 2001 census favoured such an inquiry, since it went back to the traditional questions about religious affiliations. METHODS - The ”sensitive” questions ot the census were accepted without any obstacle in the whole population and backed up the present study together with the newly established religious atmosphere in the hospital of roman catholic Hospitaller Brothers of St. John of God, which was reopened on the 1st of July, 2000. The collection of data started in November 2000 ending in December 2001. All 98 full time employed physicians participated in the inquiry carried out by personal interviews guided by the same 51 questions. Following more than half a century span, the aim of the present study was to gain measurable information about the doctors’ unknown religious affiliation and its impact on clinical problems with ethical dimensions. RESULTS - All the doctors cooperated willingly in face-to-face interviews. There was a high proportion of religious affiliation (83 persons out of 98) without any sign of mysticism in the professional values. However, the pre-eminent role of psychological factors in the healing process was stressed, opposed to the simplified materialism of medicine. The same ideological pattern was represented while ranking other professions in the social hierarchy. No physicians experienced a conflict between their religious belief and professional activity in the dayto- day service. CONCLUSION - According to the extremely underpaid medical profession in Hungary, doctors of the new hospital do not believe that now they should tolerate their underpaid status as altruistic missionaries. However, they know exactly that balancing professionalism and business of medicine, is not a task for a single hospital but an urgent one of the whole society.]

[Possible protective factors in the wound healing in light of survey]

[Purpose of the study: to investigate the correlation between the surgical procedure and the disorders of wound-healing, furthermore to examine the impact of the matter of suture on the incidence of wound healing problems. Methods and material: The survey was carried out at Surgical Clinic of the University of Pécs, among patients between 18 to 85 years who had been undergone an elective abdominal surgery between 2009-2010 (n=105). In the course of the document analysis the authors reviewed socio-demographic data and the preoperative and postoperative parameters. The authors applied descriptive statistics with SPSS 17 moreover two-sample t- test, khi square test and logistic regression (p<0,05). Results: The incidence of wound-healing disturbance is higher in the case of using draine, however it is lower if Triclosan suture is used (p<0,05). Conclusion: A number of Hungarian and international study reports that many factors can be responsible for surgical site infections. The results point out the significance of Evidence Based Nursing wound attendance in the context of team work. ]
