[In the first step the work defines the institution of advance directive, places advance directive in the context of advance care planning, and demonstrates the advantages of advance care planning. Then the forms of the advance directive are presented and related Hungarian health care sections are interepreted in the context of these back-ground conditions. In the third step decision making capacity is defined, its standards are set out, the role of substituted decision maker is delineated. In the fourth step accumulated experience with living will is demonstrated and the possible alternatives concerning the future of living will are provided: the elimination of the living will part of the institution from health care or its continued application after substantial revision. METHODS - Analytic presentation after systematic review of the literature on the topic. RESULTS - Significant experience has been accumulated since the implementation of advance directive. In light of experience two attitudes have been chrystallized regarding the future of the institution: one takes the side of the elimination of the living will part of the institution from health care, the other recommends its revised application. CONCLUSION - The American and Hungarian experiences with advance directive recommend the review of the Hungarian institution of advance directive. ]

[Cardiovascular screening and risk stratification]

[Cardiovascular diseases are still leading cause of death in developed countries. A possible background, atherosclerosis, might appear even in childhood, and deteriorate during the years without any symptoms. The cardiovascular prevention is a complex activity, which can be observed in population or/and in individual level. The conventional screening means the assessment of higher cholesterol level, blood pressure or other risk factors in healthy population without symptoms. The point of clinical risk evaluation is to find patient with high cardiovascular risk and consequentially with the necessity of intensive preventive strategy. For the right cardiovascular screening and adequate evaluation of the risk, it is needed that the doctors who frequently meet with the population execute the above-mentioned risk assessment. Therefore doctors, nurses, and education for preventive aspect would is needed, as education is important to decrease cardiovascular mortality for those patients that were categorized as a high-risk group.]

[In the first part of this review the definition of vertigo/dizziness was discussed. The major difference between the two signs is the exsistence of the direction, which is specific for vertigo. Dizziness is a frequent complaint in the clinical practice. Its frequency is increasing with advance of age, to intimate the play of declining cognitive process in the pathogenesis of its. The popular health significance of vertigo is in the rowing number of the patients. The onset of the most cases with acute vertigo appears between secundums and minutes so the patients will be provided in circumstances of emergency department. First of all three form schould be take into account: neuronitis vestibularis, benign paroxysmal positional vertigo and Meniere syndrome. Without tipical periferal signs of vertigo, central cause should be searched, principally stroke (lysis possibility). The differential diagnose of the different dizzeness/vertigo forms according to the elapsed time of the onset or congenital and acquired nystagmus was created in tables. The recommendations of the therapy of acute and chronic dizziness/ vertigo syndroms are, lack of results of evidence based trials doubtful. The more often used drugs based on clinical trials are discussed as vinpocetine, betahistine and piracetam. The in vitro and in vivo data suggest that the last molecule is eligible to use both in periferal and central type of vertigo syndroms.]

[New opportunities for the estimation of health loss in Hungary]

[BACKGROUND - The Global Burden of Disease Study has been publishing its reports since the early 1990’s. The last one from 2010 covers 291 diseases, and 67 risk factors for 187 countries. AIMS - Presenting examples of the Hungarian health burden status the authors advocate the use this unique data source. METHODS - The authors downloaded the 1990 and 2010 data for Hungary from the Global Burden of Disease Study website. To quantify the economic burden they calculated the change in GDP related to the absence from work as a consequence of risks and diseases. RESULTS - In the publication the authors presented some of the versatile utilization methods of the GBD data and the data visualization tools available on GDB’s website. In 2010 the DALY for Hungary was 3,714,900 years which is the 84% of the country’s DALY in 1990. The most Years Lived by Disability was caused by musculoskeletal disorders (YLD: 28.4%), while cardiovascular and circulatory diseases caused the most Years of Life Lost (YLL: 40.3%). In 2010 the dietary risk was the leading risk factors in Hungary accountable for 840 thousand of life years lost which economic cost was over 950 milliard Hungarian forints. CONCLUSIONS - The use of the result provided by the Global Burden of Disease Study can significantly contribute for planning and evaluating interventions to promote healthy lifestyle, to advance supporting living conditions, and to distribute health care capacity efficiently.]

[Significance of MR-angiographic technical parameters and the contrast material in the diagnosis of peripherial vascular diseases]

[Atherosclerosis presents as a significant problem in everyday healthcare. Thus far, its effect on the vascular bed was measured by means of digital subtraction angiography. More recently, due to advance in hardware, the less invasive techniques like the ultrasound, CT and MRI have into the practice, producing results very similar to those of digital subtraction angiography. Recently, contrast material aimed for blood-pool MR angiography has been introduced. This change presents as a challenge to the operating personnel since the injection rate and the MR sequence parameters need to be adjusted adequately. Also, there is a late-phase breakdown of the contrast material. A further challenge is to interpret the steadystate images. In this article an effort has been made to summarise the basis of MR angiography, with special emphasis on peripheral angiography and suitable contrast materials. Finally, we illustrate our parameters through concrete cases.]

[PRINCIPLES AND NATIONAL REGULATIONS OF BLOOD DONOR QUALIFICATION]

[During the assessment of blood donor candidates the physician considers two factors; first, whether the loss of 450 ml blood would be of any risk for them (e.g., because of hypotension) and second, whether they have any illness, current (seasonal allergy, antibiotic use, etc.) or chronic conditions (oncological or autoimmune disease, drug use, etc.) that may confer risk to the recipient. For the safety of blood preparations it is essential that the donors are dependable individuals who lead a lifestyle of low risk of getting infected (by HIV, hepatitis, etc.). Hungarian practice concerning donor qualification are generally stricter (e.g., because of the differences in the health care system, in the health culture) than the directive of the European Union. This implies that a number of donor candidates are temporarily or permanently disqualified. Following medical interventions (e.g., surgery, transfusion), environmental effects (e.g., radiation exposure) and recovery from diseases, however, the donor may again give blood after a certain period of time. Certain chronic diseases, if properly managed and if the patient is in perfect general condition, do not constitute a cause for exclusion either. General practicioners can greatly contribute to safe national blood supply by identifying and advising potential blood donors.]

[Intersphincteric resection for low rectal tumors]

[The technical development of rectal surgery, together with chemo- and radiotherapy, improved the effi ciency of surgical intervention as well as the patients‘ survival and quality of life. The treatment of ultra deep malignancies - 2-3 cm from linea dentata - is a real challenge for rectal surgery. Before the introduction of laparoscopy the removal of the deep tumors with conventional surgical techniques was impossible to save the tumorous rectum. The new techniques made possible the ultra deep resection, i.e. the removal even those tumors which progressed until the linea dentata. The explosive use of laparoscopy and the new waves of chemo- and radiotherapy resulted signifi cance advance contributed to an interdisciplinary therapeutic approach, which is a well adapted method in deep rectal surgery. In case of ultra deep localization of tumors - 0.5-1.0 cm to linea dentata - the neoadjuvant therapy supported the saving the anal sphicter. This new techniques contributed to the preservation of feces continentia of the patient.]
