[The impact of telemedicine on the development of doctor-patient relationship based on interviews conducted among physicians]

[Introduction - The doctor-patient relationship has always been an essential part of health care, however, in parallel with the integration infocommunication technologies in health care the doctor-patient communication is also transforming. Therefore, the aim of the study is to examine the effect of telemedicine on this relationship. Methods - During the study, we applied qualitative research methods and a total number of 58 semi-structured interviews (45 men, 13 women) were conducted among physicians having experience in telemedicine. The majority of the interviewees were radiologist, general practitioners, and internists. The interview questions concerned that what characterises the doctor-patient relationship in telemedicine. Results - The interviews pointed out that in teleradiology the doctor-patient relationship depersonalises and almost terminates. In this respect, the problem is often the incomplete clinical information about the patient. In turn, telemonitoring can bring a quality change in the doctor-patient communication and through remote contact the patients’ satisfaction, the sense of security, and the doctor-patient relationship will be further enhanced. Conclusions - In accordance with the academic literature - based on the research results - there is no clear evidence that telemedicine would affect doctor-patient communication only positively or only negatively. In some areas of telemedicine, this relationship is reducing (e.g. teleradiology) while in other areas it could be fur-ther strengthened (e.g. telemonitoring).]

[Rapprochement to medicine: crossing paths between Franz Alexander and Michael Polanyi]


[Franz Alexander and Michael Polanyi, who were childhood friends, schoolmates at the grammar school as well as at the medical school are renowned scientists, who were dealing with the issue of reciprocity between knower and known. Interrelation of their careers is hardly researched. In this paper, I attend to this unexplored topic. Alexander adopted the Freudian position at the age of 29. Polanyi first had turned into a physical chemist and then, at the age of 45 his calling was in the philosophy of science. I will show in this paper that their achievements are deeply rooted in the culture that they acquiered in their medical studies in Hungary. ]

[Methods of using telemedicine devices (smartphones, tablets) during home blood pressure measurement]


[Despite the use of advanced antihypertensive drugs, patients with hypertension are still insufficient to reach the target blood pressure. In this area, home blood pressure measurement was a significant step forward, but it is not at all how the method of home blood pressure measurement is done, on the other hand, it needs to be much closer to the doctor-patient relationship. [Telemedicine care for high-risk hypertensive patients with antihypertensive for reaching better blood pressure target ratio and smaller blood pressure variability]

[Telehealth care of high-risk hypertensive patients for a better target blood pressure and smaller size blood pressure fluctuation. Authors conducted a one-year, multicentre, prospective, observational study with no intervention. Their aim was to achieve better targeting and smaller blood pressure fluctuation in patients with high risk hypertensive patients with the new type of doctorpatient co-operation and telemedicinal care than the conventional control method. In the active group, 50 patients, in the non-active group 47 treated hypertensive patients were analysed. During the observation period (1 year) the mean (± SD) of the systolic blood pressure decreased from 143.3 (15.1) mmHg to 134.5 (9.2) mmHg in the active group, but in the non-active group there was no significant decrease. The target blood pressure (< 140/90 mmHg) could be increased from 46% to 62%. The two characteristics of interpersonal visit-to-visit variability index. The standard deviation (SD) and variation coefficient (VC) showed a significant decrease in the active group. There was no change in the inactive group. A method supported by telemedicine-assisted and better patientphysician- assistant co-operation is suitable for increasing target blood pressure rates and reducing blood pressure fluctuation.]

[Exploration of quality of life goals in rehabilitation of persons after spinal cord injury]

[Background and purpose - Surveyed references indicate improvement of rehabilitation outcomes if based on exploration of personal rehabilitation goals or quality of life goals. Purpose of our study is mapping of quality of life goals of persons entering rehabilitation by structured interviews, and study of applicability of the method. Methods - Structured interviews have been performed with persons consecutively admitted for their first rehabilitation intervention, meeting inclusion criteria. World Health Organization’s quality of life measure brief version and disability module served as guideline for the interviews. For study any negative effect on rehabilitation of our approach we have compared rehabilitation outcomes of consecutively admitted persons immediately before starting our study (58) and those with closed rehabilitation intervention (53). For this purpose the Functional Independence Measure was used. Finally content analysis of the interviews has been performed. Results - In 24 cases complications and psychological instability have prevented the inclusion, seven persons did not agree into participation in the study. Of the 71 interview participants the rehabilitation of 53 persons concluded. Their rehabilitation outcomes are better as of the control group significant difference has been measured in the social functions domain. Quality of life of included persons decreased during the rehabilitation process insignificantly, the autonomy domain showed insignificant improvement, the inclusion domain did not change. Content analysis revealed the outstanding role of the family among quality of life goals. Conclusions - Reported results indicate that exploration of quality of life goals of persons in early phase of rehabilitation after spinal cord injury is feasible. The selected quality of life measure may well support the interviews.]

[The role of telemedicine is to help the optimal patient-doctor cooperation in the treatment of hypertension]

[Telemedicine is now an indispensable part of healthcare and has overtaken the development of hospital information systems. Modern basic and specialized care requires the development of a state-of-the-art communication system between the patient and the physician. One of the main reasons for this is the attempt to raise the level of disease to a higher level, increase the level of prevention and care, involve the patient in the treatment, and care process as an active participant. The latter as a demand is growing ever since the various forms of telecommunication are becoming more and more informed. The technical solutions of telemedicine appear in two forms: In one, the signs and the values of the medical devices (sensors) through the various telecommunication systems are transmitted to the physician. The other solution is using smart phones and other devices (iPad, tablet), where patients send signals, data, and symptoms to your doctor. However, patient information (eg diet, lifestyle, etc.) and instructions from your doctor may arise, a continuous medical consultation can be established in which the patient is a doctor’s partner. This solution is promoted by the rapid spread of telecommunication tools in all layers of society. Authors present the methods of both solutions and details the practical aspects of telemedicine methods in hypertension disease.]

[The present and future of hypertension-care in Hungary]


[Hypertension is a population disorder of a kind that basically affects the health status, morbidity, mortality, quality of life of the population. The decrease complications its prevalence is favorably influenced by the correct treatment -defined by guidelines-, effective care, reaching and its sustained maintenance of target blood pressure, successful patient-physician co-operation. The importance of cooperation between the patient- physician- assistants- pharmacist is emphasized by numerous international organizations. The general quality of the health care system, the extensive use of the unified information system and the modern tools of telemedicine are increasingly important. The degree of drug adherence is also extremely important. According to our own experience, 40% of patients did not take the prescribed drugs in the second to third months of therapy. The Home Blood Pressure Measurement, Blood Pressure Monitoring as well as the patient education and drug revenue check mediated by smartphones, increase adherence. Real progress would mean that, like the care of heart failure, hypertension outpatients clinic (within the university centers, and the hospital departments of the county) would provide the therapeutic controll and care of the hypertensive patients.]
