[Current issues in oral anticoagulation treatment]

SAS Géza

[Cardiovascular disease is the leading cause of death in civilised countries everywhere. The situation is particularly serious in our country: it is well known that we are one of the countries with the shortest life expectancy in Europe, precisely because of the devastating impact of circulatory diseases. While in the USA, thanks to improvements in living conditions and effective preventive measures, mortality from cardiovascular disease is falling dramatically and is being postponed to later and later years, here at home we can unfortunately expect the situation to deteriorate further in the years ahead.]

[The importance of oral symptoms in the early diagnosis of HIV infection (AIDS)]

BÁNÓCZY Jolán

[Oral lesions of AIDS might occur in every phase of H/V-infection, their importance in the early diagnosis and treatment is significant. The most frequent oral symptoms are: oral Candidiasis, acute necrotizing ulcerative gingivitis (ANUG) and periodontitis, herpetic infections, resistent to the usual therapy. ,,Hairy leukoplakia " of the tongue is characteristic hyperpigmentation of the oral mucosa, Kaposi's sarcoma are frequent symptoms, latter especially in more developed phases (ARC or AIDS) of the disease. Saliva-containing a factor inhibiting HIV-1 infection of human lymphocytes - has minimal importance in the transmission of the disease. Therefore, professional transmission of HIV-infection -, e. g. dental treatment of infected patients keeping the necessary cautions , - present minimal risk. The appropriate treatment of arai lesions of HIV-infected patients may help in maintaining their general health. ]

[Family medicine, home care, family care]

ARNOLD Csaba

[General Practice as an independent speciality of medicine has a very special territory, the family care. The family care is a general wish of the society. This special care is continuously made in every fifth-sixth family. ln the author's practice in the autumn of 1989 113families and 16 lonely persons, in 1990 120 families and 18 lonely persons were controlled regularly at home. The family care, the family medicine needs special knowledge and skill. Minimal addicional technical, personal and financial support is requested, burt due to the special care activity the level of medical work increases and it becomes more humanistic, needs of hospitalization decreases and the plus expenditure refunds. Far the better evaluation of family care a more systematic undergraduate and postgraduate teaching and a special research work would be needed.]

[Mental and physical health in light of health behaviours among sexual and gender minorities]

GUBÁN Zsuzsanna, CSEKÔ Csilla, SZÉL Zsuzsanna

[In a large proportion the reason behind enlisting both primary and secondary care are psychophysiologic disorders or symptoms. In this current study we reviewed the literature regarding the physical and mental health of a subpopulation (sexual and gender minorities) and their patterns of health behavior. The literature examined in our study contains publications from January of 2009 to March of 2019. Based on the minority stress theory and the biopsychosocial model, the people from sexual and gender minorities face increased level of stress which affects their health and health behaviour in a negative way. Because of the factors mentioned above this group is more exposed to certain illnesses than the majority society – such conditions are cardiovascular diseases, lung diseases and several types of tumors and cancer. Furthermore the excess stress affects their psychological wellbeing and mental health. Summarizing the literature regarding the health and health behavior of people belonging to sexual and gender minorities our aim was to create an inventory of information for health care workers to help enhance the quality of care the members of these minorities receive. It could also support developement of cultural competence for the circle of professionals in education, law enforcement agencies, social care and health care system, in additional it might decreas health inequity.]

["Punishment-therapy” - chances of psycho-rehabilitation for mentally ill offenders under forced medical treatment]

BACSÁK Dániel, KRÁMER Lili

[When examining the life course of mentally disordered offenders it is unavoidable to take into consideration the legal definition of insanity that exempts an individual from ordinary punishment in the given context of criminal law. As technical as it is, legal language describes and prescribes institutional responses on how to deal with mentally disordered offenders - not being independent from the everyday societal stereotypes on mental illness. In Hungary, the definition of criminal liability consists of medical and legal elements. Thus, in practice court appointed psychiatric experts are solely relied upon in determining whether or not the accused are criminally liable - the formal decision is in the hands of the court. If no criminal liability is determined by the experts, the court has to acquit the accused. In some special cases this acquittal opens the way to criminal psychiatric detention that is maintained by the Hungarian Prison Service. The aims of criminal psychiatric detention are twofold: rehabilitation and punishment. We suggest that it is nearly impossible to serve both of the aforementioned aims simultaneously. Furthermore, our article argues that the philosophy of care is focused on punishment and biomedical treatment nowadays, rather than rehabilitation and holistic bio-psycho-social treatment. The approach of treatment in operation makes successful recovery of patient-detainees difficult. Moreover, there are systemic issues as well: limited effects of review proceedings, holes in the psychiatric-social institutional care system and the general societal stigmatisation of people with mental illnesses can unreasonably prolong discharging “guilty patients”, thus, they stay detained 4-5 years longer. ]

[About the same thing elsewhere, in a different tone]

ANDRÁS László

[It was a gesture of thanks and appreciation that a few days after the publication of the 1992/6 regulation on general practitioners, senior staff from the Ministry of Public Welfare and the OTF presented and discussed it with their colleagues at the LAM Club. Naturally, this was done only after the regulation had been published. They did not receive the same recognition and thanks when they made the same gesture to their colleagues sitting as Members of Parliament in the Committee on Social Affairs and Health. ]

[Medical aid in dying or physician assisted suicide]

KŐMÜVES Sándor

[The paper reflects upon the process in which physician assisted suicide, as it is traditionally called, is being reinterpreted. First, changes in word usage shall be demonstrated by examples taken form the relevant US laws and statements of prestigious US organisations. Then the interpretive possibilities of suicide, as it is traditionally called, and their consequencies are discussed. In this context the present state of art of the Hungarian criminal law is briefly presented. Finally some remarks on the normative element of the practice are made. ]
