[STUDY OF PARAOXONASE ACTIVITY IN MIXED CONNECTIVE TISSUE DISEASE]

[INTRODUCTION - Mixed connective tissue disease (MCTD) is an inflammatory autoimmune disease with multiple organ involvement. Immune- inflammatory processes play a crucial role in the pathogenesis of atherosclerosis. The connection between inflammatory parameters and atherosclerosis in MCTD has not yet been studied. Lipid abnormality is an important risk factor of atherosclerosis. Among the lipids, HDL is protective, which is in part due to the antioxidant effect of paraoxonase. In this paper, the lipid profiles and paraoxonase activities of MCTD patients were studied and the factors causing abnormalities were investigated. PATIENTS AND METHODS - Thirty-seven patients with MCTD, who had not taken any lipid lowering drugs in the past 2 months, were enrolled in the study. Thirty healthy individuals served as controls. At the time of the study the mean age of the MCTD patients was 51.2 ± 9.5 years, and the mean disease duration was 11.0 ± 7.2 years. Paraoxonase activity was determined by spectrophotometry, lipid profiles were determined by a Cobas Integra 700 Analyser, the von Willebrand factor antigen (vWFAg) was measured by turbidimetry in platelet-poor plasma and the thrombomodulin and anti-endothelial cell antibody (AECA) measurements were carried out by ELISA methods. RESULTS - Paraoxonase activity in the MCTD patients was lower than in the control population (118.5 ± 64.6 U/l vs. 188.0 ± 77.6, p<0.001). The arylesterase activity was also significantly lower in the patients (p<0.001). The reduction of paraoxonase activity was in correlation with the age of the patients, the duration of the disease and with vascular (eye, cardiac, cerebral) disorders. The total cholesterol and triglicerid levels of the patients were significantly increased compared to the control group, while in the apoA1 levels a significant reduction was seen. A very strong correlation was observed between the reduction of paraoxonase activity and the increase of endothelial cell activation markers (thrombomodulin, vWFAg, AECA). There was no difference in the values of patients with or without corticosteroid treatment. CONCLUSIONS - The results suggest that in MCTD there is an increased risk for atherosclerosis. Apart from an elevated cholesterol and triglicerid level, a reduced paraoxonase level and activity may also play a role in the development of atherosclerosis,. Therefore, in patients with MCTD, due to the increased oxidative processes and the impaired elimination of free radicals, a sustained damage to the endothelial cells occurs, which is indicated by increased levels of thrombomodulin, vWFAg, and anti-endothelial antibody.]

[’THE CYCLE OF VIOLENCE’ IN THE LIFE OF ALCOHOLICS]

[INTRODUCTION - ’The cycle of violence’ have long been a known phenomenon. The present research is aimed at answering the question of whether abuse suffered in childhood creates a tendency to aggressive behaviour in adulthood and whether there is a connection between these two forms of behaviour. METHODS - The sample studied comprised 235 clinically treated alcoholics. The instruments used for the investigation were the European Addiction Severity Index (EuropASI), the Buss and Perry Aggression Questionnaire, and the Janus Questionnaire. RESULTS - The most important finding is that persons who were physically abused in childhood by their parents were very likely to strike or beat someone in the course of their lives (χ2=9.79, p<0.001). Within the most aggressive group, 18 % had not suffered abuse in childhood, while 81 had been abused (χ2=13.25, p<0.001). If the patient had been physically abused, struck or beaten, that person later abused, struck or beat someone else (Pearson r=.397). CONCLUSION - The results draw attention to the importance of preventing and treating aggression in alcoholics undergoing clinical treatment.]

[The authors summarize the facilities of the telemedicine and telemonitoring system. The methods of telemedicine and their combination with home blood pressure measurement or the help of a nurse or pharmacist are reviewed. In the light of the latest results the authors are led to the conclusion that the intensive spread of using the facilities of telemedicine is necessary in the present-day Hungarian healthcare system. At the same time it is also determined that the methodical and technical potential is not enough to further enhance the efficiency in itself. The personal contact and the possibility of interactive monitoring from distance are definitely crucial for the continuous maintenance of the reached target blood pressure in patients suffering from hypertension and also for the augmentation of patient adherence regarding applying medicine.]

[Digitally-assisted treatment planning in precision oncology]

[The progress of molecular information based on personalized precision medicine has reached a new milestone. Actually, about 6 million mutations of 600 genes may be related to the development of cancer, and on average, 3-4 of these “driver” mutations are present in each patient. Due to the progress in molecular diagnostics, we can now routinely identify the molecular profile of tumors in clinical settings. By clinical translation, there are actually available more than 125 targeted pharmaceuticals and hundreds of such therapies are under clinical trial. As a result, we have many first-line and licenced treatment options to be elected by molecular information as the optimal one for every patient. There is an increasing need for complex informatics solutions by medical software. Geneticists, molecular biologists, molecular pathologists, molecular pharmacologists are already using bioinformatics and interpretation software on their daily work. Today, online digital tools of artificial intelligence are also available for physicians for assisted treatment planning. Telemedicine, videoconferencing provide solutions for interdisciplinary virtual molecular tumor boards, which democratizes the access to precision oncology for all doctors and patients. ]

[Methods of using telemedicine devices (smartphones, tablets) during home blood pressure measurement]

[Despite the use of advanced antihypertensive drugs, patients with hypertension are still insufficient to reach the target blood pressure. In this area, home blood pressure measurement was a significant step forward, but it is not at all how the method of home blood pressure measurement is done, on the other hand, it needs to be much closer to the doctor-patient relationship. This is the goal of introducing telemedicine tools and methods into the every day clinical practice. Three methods are known: In the first one the user uses a special application on the smartphone and writes your data into the smartphone. Solution 2 is that data are automatically transferred from the measuring device into the smartphone in the third, the smartphone itself performs the measurement. Really, the first application can be disseminated with a broadly compatible doctor-patient collaboration strategy.]

[The present and future of hypertension-care in Hungary]

[Hypertension is a population disorder of a kind that basically affects the health status, morbidity, mortality, quality of life of the population. The decrease complications its prevalence is favorably influenced by the correct treatment -defined by guidelines-, effective care, reaching and its sustained maintenance of target blood pressure, successful patient-physician co-operation. The importance of cooperation between the patient- physician- assistants- pharmacist is emphasized by numerous international organizations. The general quality of the health care system, the extensive use of the unified information system and the modern tools of telemedicine are increasingly important. The degree of drug adherence is also extremely important. According to our own experience, 40% of patients did not take the prescribed drugs in the second to third months of therapy. The Home Blood Pressure Measurement, Blood Pressure Monitoring as well as the patient education and drug revenue check mediated by smartphones, increase adherence. Real progress would mean that, like the care of heart failure, hypertension outpatients clinic (within the university centers, and the hospital departments of the county) would provide the therapeutic controll and care of the hypertensive patients.]
