[INFLAMMATION AND ATHEROSCLEROSIS]

JAKAB Lajos

[The role of vascular wall inflammation in the pathogenesis of atherosclerosis is becoming increasingly clear. However, the causal relationship between the inflammation and the course of atherosclerosis, which begins in childhood and continues for life, is debated in the literature. The interpretation of the basic pathophysiologial essence of inflammation is also controversial. This paper summarizes the basics and various features of inflammation, the body's defensive and aversive reaction. The “acute phase reaction syndrome” is a general, immediate, non-specific defense reaction of the organism, which is strongly associated with the specific, adaptive immune response. There are inflammatory processes that are chronic from the start. When looking at the main types and functions of the arterial wall proteoglycans, it is clear that they, along with the lipoprotein receptors and HDL cholesterol, are closely connected to the process, course and characteristics of the inflammation. The arterial wall proteoglycans are definitely capable of directly and indirectly influencing the inflammatory process. The issue of a possible target of statin derivatives other than the inhibition of cholesterol biosynthesis has not been resolved. Atherosclerosis may be considered a primarily chronic individual vasculitis syndrome that involves all layers of the blood vessels, and is determined by the risk factors and by the special structure of the arterial wall. The presence of inflammation is a prerequisite to the development and throughout the entire course of atherosclerosis.]

[Practical questions regarding the use of teriparatide]

TAKÁCS István

[Teriparatide has become one of the most important drug in the treatment of osteoporosis in Hungary. Although this is not a new drug, a number of questions arise regarding its use in everyday practice. When should we use it as first-line treatment? When should we change the used therapy to teriparatide? What kind of effect can we expect after the start of teriparatide therapy? What are the potential side effects? Financing rules limit, but do not fully control our therapeutic decisions, as these are mostly based on scientific data. In this review, we summarise new and older scientific data regarding teriparatide from practical aspects.]

[Community participation in sustainable development - in the wake of community medicine]

BRYS Zoltán, HARANGOZÓ Judit, SZARVAS Hajnalka

[Sustainable development aims to secure the living conditions of the next generations. Currently it fails to achieve its goal as the human destruction of ecosystem is accelerated. Institutions of the developed countries can not control the environmental crisis. The increased environmental degradation is caused by overconsumption, which is mainly driven by the widespread consumption-culture. Failure of institutional solutions drew the attention to the empowerment of communities. Aarhus Convention has legally empowered the local communities and various scientific fields examines community participation. Community Based Mental Health Services has gathered a significant knowledge about the psychosocial processes of community participation and about the participatory-culture. According to our assumption this knowledge can be used in the field of sustainable development. Besides the empowerment of the independent, local communities, concordance, affective experiencing diversity of the members, involvement of experiential experts are all important in the operation of self-organizing, responsible, local communities. We believe that the empowerment and support of eco-conscious communities is an important, new intervention in the field of sustainable development. ]

[Attracted by science III. - My scientific life product of 55 years: what have I achieved?]

dr. RADÓ János

[Author analyses the scientific results of his own clinical research work by evaluating his product in a „traditional way”, and only after then supplemented by scientometric data (citedness). From the 148 articles which has been published in foreign language (mostly in English) in abroad, 10 was grouped into the category of “new procedure/ recognition”, 40 into “original data/significant new statement”, and 98 into “new observation (statement) in a detailed question”. He has found close correlation between the values of the publications grouped by the traditional evaluation and the corresponding numbers of citations. Author discusses in the light of his own data the evaluating role of citedness with the hope to help by this motivated young scholars to plan their own future.]
