[Epidemiology of breast cancer]

[In 1987 breast cancer occupied the 3rd place among female cancers in the world. In Hungary, 13.577 women died due to cancer in 1990, 15,4% from breast cancer. The author uses international, and Hungarian data (at the country and county level) and also the data from his own research results. Incidence of breast cancer has risen continuously and shows a wide range in different parts of the world from 14,1 %000/year to 121,2%8000/year. The 5 year crude survival rate is between 40–65% and is slowly improving. In addition to the well-known risk-factors, the author emphasizes the role of mastopathy because in his patient group of 843 cases 2,3%, but in the mastopathy group 4,1% developed cancer. Therefore, this alteration should be considered as a high risk factor. Among close female relatives of breast cancer patients breast and other cancer has occurred much more frequently, than in that of a control group. Prostate cancer occurs significantly more frequently among 1 st degree male relatives. Therefore it is necessary to study the risk factors with epidemiologic methods and to broaden screening, detection and therapy in order to reduce the incidence and mortality of breast cancer. ]

[Brest cancer screening, early diagnosis]

[There is no data in the medical literature concerning the late results of breast physical exam by paramedical personnel as a screening test. In a population screening for breast cancer in Debrecen, nearly 70 thousand women were observed from 1981 to 1985 and 298 new breast cancer patients were detected. 198 patients were from the screened population and 100 patients from the non screened population. All of the breast cancer patients were followed up, and the analysis was completed on 31st December 1991 with the help of statistical methods. The author analysed the overall survival, cumulative death rate and relative risk of dying according to age group. The difference of overall survival was 29% for the screened group. This result is nearly the same as that produced by mammographic screening.]

[Diagnostics of breast cancer]

[The author interpretes and evaluates the diagnostic methods of breast cancer. Detailed description is given on physical examination, mammography, ultrasound examination, cytology, histology and miscellaneous methods, expressing their advantages and drawbacks. On clinical experience nowadays the combinative application of these by the same expert seems to be the most effective method, possibly in large breast clinic centres.]

[Breast cancer markers of prognostic significance]

[Prognosis prediction of breast cancers is difficult, particularly in early clinical stage (1) or in tumors with practically identical histological structure and degree of differentiation (invasive ductal NOS cancer) because node negative tumors of early developmental stage or with identical structure may demonstrate different clinical course. In such cases the steroid hormone receptor content, the prolife rating capacity of the tumor and the so called independent prognostic factors like the onco gene and supressor gene expression (c-myc, C-erbB–2, p53), the growth factor receptor content (EGFr) and the so called differentiation antigens accompanied by low metastatising capacity (MAM–6, nm23) may help the pathologist in diagnostics. Examinations of these markers are planned to be introduced in future diagnostics and contribute to the elaboration of effective treatment schedules.]

[The surgery of breast cancer]

[The incidence of breast cancer in Hungary is gradually increasing, not only in the elderly due to the increasing average age, but in younger women as well. In the operative stadia (St I-II), the best results can be achieved by a radical surgical intervention and an adjuvant radio-chemo-hormone therapy. The formerly routine radical mastectomies are being replaced by breast conserving procedures which remove the tumour and sacrife only the most necessary surrounding tissues to achieve the best local tumour control. The fundamental basis for this procedure was established by the clinical trials of B. Fisher and U. Veronesi. The practical basis is the circumstance, that owing to better propaganda, more frequent breast self examinations, and better mammographic and cytological facilities, more breast cancer will be recognized in an earlier stage.]

[THE INFLUENCE OF GASTROESOPHAGEAL REFLUX DISEASE AND ITS TREATMENT ON ASTHMATIC COUGH]

[INTRODUCTION - Gastroesophageal reflux is known to cause chronic cough and it is also implicated in worsening of asthma. We conducted a prospective study to examine the clinical significance of gastroesophageal reflux disease in asthmatic patients with chronic cough, to analyse the temporal relationship between reflux events and coughing and to assess the effect of esomeprazole treatment on respiratory symptoms and lung function in these patients. PATIENTS AND METHODS - 126 asthmatic patients with chronic dry cough were studied. Diagnosis of gastroesophageal reflux disease was based on typical symptoms and the effectiveness of therapeutic test or on pH monitoring, while control group consisted of the patients without gastroesophageal reflux (negative pH results). The study group patients received the proton pump inhibitor esomeprazole (40 mg/day for three months) and standard treatment for asthma was continued. During the study pulmonary function tests (forced expiratory volume in one second and peak expiratory flow) were evaluated four times and the reflux symptom scores as well, using a questionnaire. RESULTS - The results of pH monitoring showed that 64% of cough episodes were related to acid reflux and in 91% of reflux events preceded coughing. Esomeprazole treatment (40 mg/day for three months) not only diminished gastroesophageal reflux symptoms but also improved asthma outcome measures. Baseline pulmonary function values increased significantly together with a decrease in symptom scores and the use of rescue medication. In most patients included in the extended part of the study for another three months, the dose of inhaled steroids could be reduced with sustained therapy against gastroesophageal reflux. CONCLUSION - Our data shows that reflux events preceded coughing in most cases and that treatment of gastroesophageal reflux disease caused an improvement in different outcome measures of asthma suggest that gastroesophageal reflux disease worsens asthma and its treatment is of clinical importance in the effective management of these patients.]

[Hematopoetic stem cell transplantation for pediatric non-hematological solid tumors]

[High-dose chemotherapy followed by stem cell rescue or allogeneic stem cell transplantation in the treatment of solid tumor with non-hematologic origin applied for more than three decades. High-dose chemotherapy with stem cell rescue is the part of standard fi rst line therapy for several chemosensitive tumors with unfavorable outcome (neuroblastoma, Ewing-sarcoma, medulloblastoma), and also successfully applied in certain therapy-resistant or recurrent solid tumors (germ cell tumors, Wilmstumor). Allogeneic transplantation with reduced intensity conditioning is still not successful in terms of survival in pediatric solid tumors with non-hematological origin. In present paper results of autologus and allogeneic stem cell transplantation in different pediatric solid tumor are discussed.]

[Confined placental mosaicism and uniparental disomy]

[Chromosomal mosaicism is defined as the presence of two or more cell lines having different chromosomal complements in the same individual. In the conceptus the extent of the mosaicism depends on the timing of chromosomal mutation occurance, the cell lineage affected, and the viability of the mutation. The resultant mosaicism can be either generalized, confined placental or confined embryonic. The process of the loss or removal of one of the three chromosomes from the trisomic conception, at least from the cells that will form the proper fetus is known as trisomic zygote rescue. As the result of this phenomenon, the embryo/fetus becomes disomic, while the placental compartment remains trisomic or mosaic. After losing a chromosome, the remaining pair might originate from the same parent. The presence of two chromosomes from one parent in a disomic cell line is termed uniparental disomy. Uniparental disomy is one form of aberrant origin for disomic cells, and the term „pseudodisomy" is also used. Uniparental disomy can involve homozygosity for the chromosome, and the term ,,isodisomy" has been suggested for this phenomenon. If the homozigosity for the chromosome is not complete, the term „heterodisomy" is used. Depending on the pathologic chromosome, the clinical consequences of the confined placental mosaicism and uniparental disomy can be intrauterine and/or postnatal growth restriction, spontaneous abortion. Increased perinatal morbidity and mortality, minor congenital malformations can result from the phenomena. Confined placental mosaicism and uniparental disomy are well known in syndromatology too. The connections of mosaic trisomy 7 and Silver-Russell syndrome, mosaic trisomy 15 and Angelman syndrome, mosaic trisomy 15 and Prader-Villi syndrome are described. Due to the presence of aneuploid cells in the placenta, confined placental mosaicism may cause placental dysfunction, hydropic degeneration of the placenta or „unexplained" highly increased serum hCG level. ]

[The psychological preparation of Zala Special Rescue Team for operating during crises]

[The experiences of interventions in disasters and crises of the last decade evidenced that in addition to the professional state forces specially equipped and mentally prepared volunteer and professional rescue groups were necessary operated. The goal of this article is to demonstrate the importance of psychological preparation for helpers and the risks of its hiatus during an intervention. The preparation of Zala Special Rescue Team is presented in our article along with its results. Authors’ review in connection with the rescue team members, their stress pressure, and the possible strategies was based on their long-term experience and relevant literature. ]

[Nurses Knowledge and Tasks of real Time Evacuation of Inpatient Care Institutions]

[In our modern, globalized world, there is a significant increase in the number of events requiring immediate action where coordinating the rescue, health care and placement of several people at the same time. Unfortunately, we can count not only on international, but domestic relations, for various mass disaster threats and events, whether it is a natural disaster or a different form of artificial disaster. It is of the utmost importance to treat such a qualified situation in inpatient care and health care institutions, where there is a large number of patients and caregivers with limited self-reliance and movement. Although it is important to emphasize the importance of disaster prevention in the domestic context, in cases requiring immediate and rapid intervention, there is a great deal of responsibility and responsibility for nursing staff. By questionnaire surveys, we investigated the catastrophic and evacuation knowledge of inpatient care nurses and the system and method of acquiring these skills. After the results were processed, we concluded that our assumptions proved that the knowledge of the target group in question was very incomplete. In order to increase efficiency, we consider it of the utmost importance to develop a procedure based on unified principles but to adapt to the local conditions of the given institution and to develop its ability-based education system. ]
