[Postmenopausal changes of immune system-related genes expression in human bone tissue]

[INTRODUCTION - The molecular and cellular interactions between the immune system and bone tissue have been established. Sex hormone deficiency at the time of menopause has multifunctional role by influencing growth, differentiation and metabolism of the skeletal and the immune system. We have used non-parametric and multidimensional expression pattern analyses to determine significantly changed mRNA profile of immune system-associated genes in postmenopausal (POST) and premenopausal (PRE) non-osteoporotic bone. MATERIALS AND METHODS - Ten bone tissue samples from POST patients and six bone tissue samples from PRE women were examined in our study. The transcription differences of selected 50 genes were analyzed in Taqman probe-based quantitative real-time RT-PCR system. RESULTS - Mann-Whitney test indicated significantly down-regulated transcription activity of 3 genes (CD14, HLA-A, ITGAM/CD11b) and upregulated gene expression of 6 genes (C3, CD86, IL-10, IL-6, TGFB3, TNFSF11/RANKL) in POST bone. According to the canonical variates analysis results, the groups of postmenopausal and premenopausal women are separable by genes coding for cytokines, co-stimulators and cell surface receptors affected in antigen presentation and T cell stimulation which have high discriminatory power. CONCLUSIONS - Based on a complex gene expression patterns in human bone cells, we could distinguish POST and PRE states from an immunological aspect. Our data might provide further insight into the changes of the intersystem crosstalk between immune and skeletal system, as well as local immune response in the altered microenvironment of POST bone.]

[Carvedilol and its antioxidant effect]

[Carvedilol, the typical basic variant of the third generation beta blocker drugs is a complex adrenergic blocker that also has Ca channel blocking effects. It has no effect of the metabolism and has a pregnant antioxidant effect that is significant for cardiac and hypertension target organ protection. Its beneficial effect on cardiac decompensation, on target organ protection in patients with hypertension and on primary and secondary prevention of ischemic heart disease is proven by clinical studies. The effect of free radicals trapping - not shown by the majority of beta blockers - plays a major role in these beneficial effects. Inflammatory factors and free radicals (ROS) play a central role in cardiovascular diseases and can be regarded as prognostic markers of vascular damage. Elevated levels of glucose, lipids, or elevated intraluminal pressure triggers the production of various free radicals. The anti-inflammatory effect of carvedilol results out of its antioxidant (scavenger) and ROS suppressive effects. Besides its complex adrenergic blocking effect, this ability of carvedilol gives a molecular explanation for its efficiency proven by clinical trials.]

[A new DPP-IV inhibitor: saxagliptin]

[Saxagliptin is a selective, potent inhibitor of dipeptidyl peptidase-IV (DPP-IV). By inhibiting DPP-IV, saxagliptin reduces the degradation of endogenous incretin hormones, resulting in increased glucose-dependent insulin and decreased glucagon secretion from the pancreas islets. Clinical trials of saxagliptin as monotherapy and as combination therapy with other oral antidiabetic medications including metformin, glibenclamide, glipizide, pioglitazone and rosiglitazone have demonstrated clinical benefits in different glycaemic endpoints. Due to its glucose- dependent mechanism of action, saxagliptin as monotherapy or in combination with metformin results in a very low risk for hypoglycemia. It has also been shown to be generally well-tolerated, with not having any relevant effect on weight. The authors summarize the most important saxagliptin trials.]

[POSTMENOPAUSAL HYPERTENSION]

[Women with normal cycles on the average have lower blood pressure than age matched men. From the fifth decade of life increasing average blood pressure values of females reach or even exceed the levels of males. The frequency of hypertension among women in menopause and postmenopause is 3-4 times greater than in premenopause. This great difference can not be explained by the age dependent increase in blood pressure. There are several pathological components in the background of the elevation of blood pressure following the reduction and failure of female sex hormone production. Among these components are lifestyle changes, reduced physical activity, growing body weight, increased sympathetic activity, higher RAS (renin-angiotensin system) influence and increased salt dependence of blood pressure seem to be the dominant factors. Contrary to earlier suggestions recent clinical findings have proved that estrogen or combined hormone replacement therapy did not increase but rather slightly reduced the blood pressure in menopausal and postmenopausal hypertensive women. Therefore, hypertension itself should not be the contraindication against carefully managed hormone replacement therapy. The therapy is frequently used for preventing severe osteoporosis in spite of the disappointing cardiovascular results of the WHI and HERS trials. In the Joint National Committee 7 there are no special recommendations which would strictly prefer one or another antihypertensive agents in the pharmacological treatment of postmenopausal hypertension. However doctors are assisted in their individual therapeutical decisions by certain clinical and experimental findings which refer to higher sympathic activity, enhanced RAS influence and increased salt dependency in the pathomechanism of postmenopausal hypertension.]

[The influence of menopause-related obesity and related changes of body fat distribution on the severity of sleep apnea]

[INTRODUCTION - The menopause is associated with an enhanced risk of obesity. During the postmenopausal period changes in the distribution of body fat lead to a variety of disorders. Obesity is among the major risk factors for Obstructive Sleep Apnea Syndrome (OSAS). The prevalence of OSAS increases after the menopause. This study was intended to explore the relationship between the severity of sleep apnea and the quantity, as well as the distribution of body fat in postmenopausal women with this condition. PATIENTS AND METHODS - Sixty-two postmenopausal women (aged 58.6±7.4 years) were studied. Patients suffering from OSAS - established by cardiorespiratory polygraphy - were enrolled. None of the subjects received hormone replacement therapy during the trial. Total and regional quantity of body fat was determined by dualenergy X-ray absorptiometry (DXA). The distribution of body fat, the ratio of android-to-gynoid regional fat, as well as body mass index were automatically calculated by the software of the DXA machine. A specific region was defined to measure the fat content of the cervical region, extending from the mental protuberance to the clavicular plane. The reliability coefficient of the test method was calculated to check the accuracy of regional body fat measurement. The severity of obstructive sleep apnea was determined by cardiorespiratory polygraphy and expressed using the apnea/hypopnea index. RESULTS - Testing for independence in this population revealed the lack of independence between android-type obesity and severe OSAS. Specifically, 74% of patients with severe OSAS were obese (BMI>30 kg/m2). As with the android-type, the khi square test similarly refuted the independence between obesity and the severity of OSAS. Fat content of the cervical region was 25.2% in mild and moderate, and 30.2% in severe OSAS. Two-Sample t-test demonstrated the significant influence of cervical fat content on OSAS severity. CONCLUSION - Elevated BMI, android-type obesity, and higher relative fat content of the cervical region all aggravate obstructive sleep apnea in postmenopausal women.]
