[Functional magnetic resonance imaging studies in pain research]

[Functional imaging studies opened a new way to understand the neural activity underlying pain perception and the pathomechanism of chronic pain syndromes. In the last twenty years several results of functional magnetic resonance imaging (fMRI) studies have been published about examining the different aspects of complex pain experience. The aim of these studies is to understand the functioning of the pain control system, the so-called pain matrix, activated by acute nociceptive stimulus. Another important field of pain research is the investigation of neuronal processes underlying chronic pain, since the pathomechanism of this is still unclear. Our review aims to provide insight into the methods of pain research using fMRI and the achievements of the last few years.]

[Mental and physical health in light of health behaviours among sexual and gender minorities]

[In a large proportion the reason behind enlisting both primary and secondary care are psychophysiologic disorders or symp­toms. In this current study we reviewed the literature regarding the physical and mental health of a subpopulation (sexual and gender minorities) and their patterns of health behavior. The literature examined in our study contains publications from January of 2009 to March of 2019. Based on the minority stress theory and the biopsychosocial model, the people from sexual and gender minorities face increased level of stress which affects their health and health behaviour in a negative way. Because of the factors mentioned above this group is more exposed to certain illnesses than the majority society – such conditions are cardiovascular diseases, lung diseases and several types of tumors and cancer. Furthermore the excess stress affects their psychological wellbeing and mental health. Summarizing the literature regarding the health and health behavior of people belonging to sexual and gender minorities our aim was to create an inventory of information for health care workers to help enhance the quality of care the members of these minorities receive. It could also support developement of cultural competence for the circle of professionals in education, law enforcement agencies, social care and health care system, in additional it might decreas health inequity.]

[Self-monitoring of blood glucose in type 2 diabetes]

[Self-monitoring of blood sugar levels is an essential element in the treatment of diabetes, it provides an insight to the success of treatment and self-management, the development of metabolic processes, and ensures the early recognition and treatment of significant blood sugar fluctuations. The aim of this study was to evaluate the habits of self-monitoring of sugar levels in patients with type 2 diabetes, and among them the frequency, recognition and treatment of hypo- and hyperglycemia. The research was conducted as a cross-sectional study in 2018 in the Health Centre in Subotica/Serbia. The sample included 107 patients diagnosed with type 2 diabetes. Data were collected through interviews with patients using a customized questionnaire, during the regular control examinations. The patients’ level of blood sugar in the capillary blood was measured by glucometer. The statistical package IBM-SPSS 20.0 and descriptive analytic methods were used for data analysis. The mean age of the sample was 63.7 ± 7.7 years. Two-thirds of them performed opportunistic self-monitoring mostly before breakfast. A large number of patients never recognized variations of blood glucose (in 65% hyperglycemia and in 51% hypoglycemia). However, a month before 81% of patients had hyper- and 31% hypoglycemic episodes respectively. 12% of patients did not take any action in such cases. It is recommended to emphasize again and again the importance and way of self-monitoring at each follow-up examination. The shortcomings identified in the study provide guidelines what to focus on when caring for diabetic patients.]

A case with reversible neurotoxicity induced by metronidazole

Background - Metronidazole is a synthetic antibiotic, which has been commonly used for protozoal and anaerobic infections. It rarely causes dose - and duration - unrelated reversible neurotoxicity. It can induce hyperintense T2/FLAIR MRI lesions in several areas of the brain. Although the clinical status is catastrophic, it is completely reversible after discontinuation of the medicine. Case report - 36-year-old female patient who had recent brain abscess history was under treatment of metronidazole for 40 days. She admitted to Emergency Department with newly onset myalgia, nausea, vomiting, blurred vision and cerebellar signs. She had nystagmus in all directions of gaze, ataxia and incompetence in tandem walk. Bilateral hyperintense lesions in splenium of corpus callosum, mesencephalon and dentate nuclei were detected in T2/FLAIR MRI. Although lumbar puncture analysis was normal, her lesions were thought to be related to activation of the brain abscess and metronidazole was started to be given by intravenous way instead of oral. As lesions got bigger and clinical status got worse, metronidazole was stopped. After discontinuation of metronidazole, we detected a dramatic improvement in patient’s clinical status and MRI lesions reduced. Conclusion - Although metronidazole induced neurotoxicity is a very rare complication of the treatment, clinicians should be aware of this entity because its adverse effects are completely reversible after discontinuation of the treatment.
