[Pregnancy and insulin resistance: theoretical and clinical issues]

[During pregnancy, metabolic changes occur physiologically, which are enhanced by the presence of gestational or praegestational diabetes. The basis of these changes is the increasing insulin resistance throughout pregnancy, which in diabetics may result in hyperglycaemia with undesirable clinical consequences and complications. These complications can be prevented in diabetic pregnant women by maintaining the physiological metabolic state typical of healthy pregnant women. Thus the aim of the treatment is to achieve a normoglycaemic state throughout pregnancy. In most cases this is only possible by the use of insulin, along with appropriate dietary measures. Intensive insulin regimes are successfully used in the metabolic control of pregnant diabetic women, and the use of insulin analogues and insulin pump may also be considered.]

[Psoriasis is one of the most common dermatological disorders. It is an immune-mediated chronic inflammatory disease that primarily affects the skin and the joints, and substantially reduces the quality of life. It occurs worldwide, with a prevalence of about 2% in developed countries. Today numerous studies are continuously published on psoriasis. Important progress has been made especially on its genetics, pathomechanism and comorbidity. In the past few years, several efficient therapeutic approaches have been developed and applied. With the appearance of biological therapies, the inflammatory process can be targeted more selectively than before. The paper reviews most recent data on the pathogenesis, immunological background, comorbidity and reduced quality of life associated with psoriasis, and presents therapeutic approaches with special focus on recent advances. The increased knowledge allows of a more efficient treatment and care of patients with proriasis, thus significantly increasing their quality of life.]

[The impaired physical activity and the related increased cardiovascular risk is caracteristic in all stages of chronic kidney diseases. [What is worth to know about COVID-19 for (not only) a cardiologist]

[SARS-CoV-2 virus infection sprang from Wuhan the capital of the Chinese Hubei province, at the end of 2019 and caused a worldwide pandemic with 1.5 million confirmed cases and claimed almost 100 000 victims until the beginning of April, 2020. First analyses of Chinese COVID-patients confirmed that diabetes, hypertension, and cardiovascular diseases were highly prevalent among SARS-CoV2 infected patients, and might be associated with poor outcome. As previously shown for SARS-CoV-1, SARS-CoV-2 similarly utilizes ACE2 as receptor for viral alveolar cell entry. A suspicion has arisen that the widely used ACE-inhibitor/ARB therapy could be potentially harmful for patients suffering from COVID-19 infection as these agents upregulate the ACE2-expressions. From the other point RAAS-blockade might be beneficial due to fact that ACE2 counters the deleterious effects of Angiotensin II. Authors provide a comprehensive over­view of the most recent literature and summarize the link between COVID-19 and car­diovascular disease. It is important to em­phasize that there are no available hu­man evidences confirming if the RAAS-in­hi­bitor therapy were harmful or helpful in pa­tients suffering from COVID-19.]

[Doctors’ obligations and patients’ rights in the health legislation of the Austro-Hungarian Empire in the 19th century]

[Health care services, particularly the in-patient care experienced a considerable development during the period of Dualism since 1867 in the Hungarian part of the Empire. Its legal basis created the first Hungarian Act on Public Health (Act XIV 1876). Nobody is questioning today the legal protection of patients as consumers of the health care services. It was regulated in comprehensive terms the first time by the 1997 health legislation (Act CLIV of 1997 on Health). Patients’ rights existed already prior to the 20th century however they were dealt with only by few studies thus far. The primary aim of our study is 1) analysing doctors’ obligations regulated by the first Hungarian Act on Public Health (Act XIV 1876) and the Hospital Decree, 2) comparing these legal norms with the actual Act on Health as well. Based on the comparative analysis of the first Hungarian Act on Public Health (Act XIV 1876) and the Act CLIV of 1997 on Health it is obviously demonstrated that endeavouring the protection of patients in vulnerable situation was clearly determined in the 19th century regulation and the patients’ rights in contemporary sense of the word were also emerging. As a result, we draw the conclusion that we must break with the frequently voiced claim about the first emerging of patients’ rights only in the 1972 health legislation in the middle of the 20th century transferred and systematically itemized later in the comprehensive Act of Health in 1997. Our today’s regulation in force was already anticipated in the 19th and even 18th century Hungarian health legislation.]

[PYCNOGENOL IN THE CLINICAL PRACTICE]

[Polyphenols, which belong to the group of flavonoids, can be found in a number of plants, and are present in a high concentration in the French maritime pine bark. The authors summarise results of large-scale experimental and clinical studies on pycnogenol, the standardised extract of French maritime pine bark. Pycnogenol decreases production and effects of free radicals (antioxidant effect). It has antiinflammatory properties, and, by the stimulation of eNOSsynthesis, it increases the production of vasodilatory compounds (e.g. NO, prostacyclin) and decreases that of vascoconstrictor compounds (endothelin-1, thromboxane) materials. These changes lead to vasodilation, which results in increased tissue perfusion and decreased blood pressure. Pycnogenol also decreases platelet aggregation and LDL-cholesterol level and increases HDL-cholesterol level. Its antidiabetic effect has also been shown. Consequently, it may substantially decrease cardiovascular risk. In addition to these results, pycnogenol has been also found to have antibacterial and antiviral effects. It has been successfully used in children with attention deficit hyperactivity disorder, as well as in adults with dysmenorrhea, climacterial disturbances, glaucoma or asthma bronchiale.]

[The cardiovascular system and the coalugation process play essential role in regulating the homestasis of the human body. Thrombuses may appear in veins (venous thrombosis) as well as arteries (arterial thrombosis) that may cause a wide range of ischemic vascular diseases. By mapping genetic risk factors that may accelarate the development of thrombosis, the quality of medical preventions and therapies can be improved. The most frequent gene mutations (FII, FV, PAI-1, MTHFR and EPCR gene polimorphisms) can be tested by methods based on PCR, real-time PCR and macroarray techniques. Professionals may use genetic results for selecting appropriate and optimal therapies based on the context of a patient’s medical history.]
