[Alzheimer’s disease and arrhythmia: cause, effect, complication]

[Alzheimer’s disease (AD) is a new endemic of the 21st century which becomes the biggest health and social problem of the ageing societies in the next few decades. Vascular factors, such as cardiac arrhythmias, especially atrial fibrillation, play an important role in the pathogenesis of AD. Arrhythmias might develop as a consequence of AD, too, and they might be caused by the cholinergic medications used in the treatment of AD. In addition, AD has a major influence on the treatment of arrhythmias, especially atrial fibrillation. Because of these, AD and arrhythmias might accompany each other in the practice of several medical specialties; these interactions are reviewed in this paper. ]

[Cilostazol in therapy of peripheral atherosclerotic patients]

[INTRODUCTION - Cilostazol is available in Hungary for patients with peripheral atherosclerotic disease (PAD) since June 2014. PATIENTS AND METHOD - In this prospective clinical data collection 68 patients were investigated with PAD-CI, treated with cilostazol. All patients were in stage of Fontaine II at the beginning of the study. Every patient has been able to control for at least three months. Two investigated groups have been treated with cilostazol. Group I: patients operated on lower extremity with femoropopliteal bypass earlier of this study and group II: patients with PAD but no arterial reconstruction in their medical record. Every patient got 100 mg cilostazol twice a day. Anticholesterol, platelet inhibition treatment have been continued in every case. No patients have taken pentoxifyllin. The end points of the data collection were the walking distance alteration (elongation) expressed in meter and the changing of the complaints of claudication intermittent. 61 patients have tolerated cilostazol treatment without side effects. Seven patients out of 68 have not tolerated it, that is why the treatment was suspended. Thirty-four out of 61 patients’ walking distance was elongated with at least 50 meters. 21 patients’ claudication improved between 50 to 100 meters and 12 patients’ walking distance improved more than 100 meters. CONCLUSION - The cilostazol presumably will be an unavoidable factor in the effective treatment of peripheral atherosclerotic disease. ]

[Five-year results of „Comprehensive Health Screening of Hungary 2010-2020”]

[The „Comprehensive Health Screening of Hungary 2010-2020” have finished the 5th jubilee season. Int he past five year the program was working succesfully as a model for the primer and secondary prevention. The results of comprehensive screening have shown caracteristic picture about the health of population, and many people received education, informations about health protection, prevention and healthy lifestyle. More than 900 places, 7 millions of filled risk queries, 112 000 people’s comprehensive screening and 250 000 counsellings are the summary of activities. Some 200 000 people received the information pack. Significant is the familiar occurence of tumors and cardiovascular diseases, that means 20% of prevalence in test subjects. We are obese, smokers, physically inactive, our health behavior is poor. The improvement of the populational health care activity is among the strategical plans of health politics, and it is badly needed based on the results of MÁESZ Program.]

[Inflammatory myofibroblastic tumor of abdominal wall arising from uterine myoma]

[INTRODUCTION - The inflammatory pseudotumor appears in various organs. It is well circumscribed, unencapsulated proliferation which simulates malignant tumor clinically as well as radiologically and morphologically. It’s etiology is unknown. The histology prooves inflammatory origin of this process: the nodular structure of the tumor-like mass is composed of fibroblasts, myofibroblasts, plasma cells and lympho­cytes. Recurrence may happen. CASE REPORT - Because of the rapidly expanding uterine myoma of a 40 years old woman laparotomy has been performed. In the lower two-thirds of abdominal wound the myoma penetrated into the abdominal wall. Here the abdominal wall lost it's normal structure, became bacon-like, homogenous in consistency and white-grey in colour. The myoma and the attached part of the abdominal wall was excised together. The part of myoma those in contact with abdominal wall contained proliferating myofibroblasts, fibroblasts and was infiltrated by plasma cells and lymphocytes. These findings could be seen in the abdominal wall too. The rapid proliferation which simulated malignant tumor has been diagnosed as inflammatory myofibroblastic tumor. The patient does well two years after the operation. CONCLUSION - It is important to know about the entity of inflammartory pseudotumor in differential diagnostic point of view, as it mimics malignant tumor. The structure is nodular, the histology is characteristic and the therapy is surgical. The follow up is very important because of possibility of probable recurrence. ]

[Amlodipine/atorvastatin fix combination in type 2 diabetes mellitus]

[INTRODUCTION - Type 2 diabetes mellitus is known to represent a major cardiovascular risk. In type 2 diabetes, hypertension and dislipidemy are often also present, increasing the vascular risk. In this case the use of both antihypertensive and lipid lowering therapies is needed. In our CASE REPORT, we discuss the data of a 43 years old diabetic man with BMI: 29.4 kg/m2, waist circumference of 101 cm, who had recently diagnosed hypertension and was given single pill amlodipine/ atorvastatine in addition to his original ACEI therapy. At the initiation of the therapy the mean blood pressure - measured during ABPM was 149/91 ± 14.99 Hgmm, his total cholesterol: 4.18 mmol/l, HDL cholesterol: 0.82 mmol/l, LDL cholesterol: 2.41 mmol/l, and triglicerid: 4.09 mmol/l. Three months later the results were the following: mean blood pressure: 121.66/ 82.79 ± 8.21 Hgmm, total cholesterol: 3.47 mmol/l, HDL cholesterol: 1.08 mmol/l, LDL cholesterol: 1.98 mmol/l, triglicerid: 1.44 mmol/l. There were no side effects. Using the therapy among several other diabetic patients we observed similar efficacy and tolerability, and the adherence of the patients was perfect. DISCUSSION - In type 2 diabetes the use of the fix combination of amlodipin/atorvastatin in addition to previous ACEI therapy is effective, well tolerated, and the long term compliance proves to be good.]
