[Basics and methods of radiostereometric analysis in orthopaedic practice]

[Photogrametry is a discipline processing geometric information of images according to the general image concept. Radiostereometric analysis (Roentgen stereophotogrametric analysis) is the most dynamically developing area of the orthopaedic photogrametry. The aim of the authors was to summarize the different methods of radiostereometric analysis in use. Two basically different methods are used: marker-based and model-based radiostereometric analysis. The authors summarize and compare the steps, the advantages and the disadvantages of either method. Radiostereometric analysis is because of its high precision appropriate as a tool in scientific research and diagnostic in orthopaedics, such as detecting the 3D micromotion of different components of implants, the wear of polyethylene inserts or determining anterior-posterior and rotational movements of the knee. Analyzing the reproducibility and accuracy of the reconstruction methods one can establish that the accuracy of analytical methods in use is behind the accuracy of digital automated software. The precision of the model-based radiostereometry can never achieve the accuracy of the marker based radiostereometry because of the inaccuracy of the initial input model of implants, although the one of the method of reversed engineering - one of model-based radiostereometric analysis - can approach the accuracy of marker-based radiostereometry.]

[Care of PAD (peripherial arterial disease) is one of the most negligent area of cardiovascular diseases: on one hand patients suffering from PAD are the most uncared, less educated and health-minded population (it’s enough to keep in mind the high rate of smoking and alcohol-abuse among them) in addition the number of angiologists is insufficient for the treatment of PAD. Although detection of PAD would be easy as a widely applicable, cheap screening method, the ankle-brachial index is available which is an obligatory screening investigation in general care. Therapeutic options are better and better since cilostazol was launched we have an evidence-based treatment which can solve the symptoms, slower the progression which is strongly recommended (IA recommendation) in both American and Hungarian guidelines. Cilostazol, a PDE3-inhibitor has complex pharmacological effect but contraindicated in several clinical conditions as pentoxiphylline is not totally excluded from the therapeutic arsenal of PAD. The place of antiplatelet agents is well-defined in the most recent European guidelines in the treatment of PAD. The jumping development of interventional radiology beyond medical therapy contributed a lot to prevent limb amputations in the case of early detection of PAD. ]

[Digital volume tomography - The use of cone beam CT in dentistry, oral and maxillofacial surgery]


[Oral and maxillofacial radiology is a subspecialty with its own field of indication. The goal is to achieve proper diagnostic image quality with the minimal amount of harmful radiation. The most common acquisition techniques are the intraoral radiograph and the panoramic radiograph which result in an overview picture of the whole dental status of the patient or the full mouth survey with the higher doses of radiation indicated for periodontological treatment. The next level is the supplementary radiograph such as occlusal radiograph, transversal tomography (some panoramic radiographs have this option), lateral cephalometric projection, submentovertex view or Waters projection, etc. More over cone beam CT acquisition or digital volume tomography as is called. In case of some described special indications CT, MRI or sometimes US acquisition can be made. In the field of three dimensional radio-diagnostics, the CT has a superior place with well-known advantages for everybody, and the usage has been limited only by the high radiation dose. The main point of the acquisition is the image quality. The load of radiation only makes the field of indication narrow. In every day practice - because of the higher radiation load of each high quality CT - the pictures passing to the doctor are preferred to take with lower resolution and wider slices although the diagnostic value of this never reaches the wanted level. This is why this new acquisition system also mentioned in the title would be better known. This system works with reasonable low radiation coupling with the possibilities of the high fidelity 3D imaging focusing on the bony structures of the head and neck region. The purpose of this article is to give a comprehensive introduction to this method in use for more than a decade. From 2006 in Hungary we also have the option to use the technology.]
