[Learnings from the local Monkeypox Epidemic]

[Monkeypox is an infectious disease that has been known for several decades and spreads from animals to humans, caused by the monkeypox virus, which belongs to the orthopoxviruses. Variola, the smallpox virus is also a member of the same family of viruses. The first human case of monkeypox was described in 1970 in the Democratic Republic of the Congo. The disease spreads epidemically in West and Central Africa, and has only occasionally caused diseases outside of Africa. The monkeypox virus also spreads from one person to another through injuries, body fluids, respiratory droplets, and through close contact with contaminated materials such as beddings. The presence and spread of the virus in Europe is a novelty. The symptoms of the disease are similar to smallpox, which the WHO declared to have disappeared from the face of the Earth in 1980. The first monkeypox outbreak outside Africa was registered in 2003 in the United States of America. Since May 7, 2022, many European countries have reported diseases caused by monkeypox to the WHO, and on May 31, zoonosis was also identified in Hungary. According to the WHO, most patients self-reported to be MSM men. The MPXV epidemic can be prevented by isolating the infected patients, following hygiene rules, properly washing one’s hands, alcohol-based disinfectants, and the appropriate use of personal protective equipment in health care. In our article, we analyse with the current monkeypox epidemic and discuss the following topics: epidemiology, diagnostics, clinic, infection control rules, and characteristics of the spread of monkeypox.]

[Investigating older People living in their own Homes’ Knowledge of Risk Factors and their daily Fall Prevention Behaviours]

[To assess the prevalence of falls, knowledge of fall risk factors and daily fall prevention behaviour of elderly living in their own homes. 173 elderly people were surveyed. In addition to sociodemographic variables, we examined the prevalence of falls, risk of falling, knowledge of risk factors, modification of living environment, and daily fall prevention behaviors. Descriptive and mathematical statistics (chi-square, t-test, correlation analysis) were used (p<0.05). 37% of elderly people had a fall in the past year and 81.2% of them had experienced an injury. Among the risk factors, modifiable risk factors were the least accepted by the elderly. The idea of creating a safe environment was addressed by 72.8% of the sample, but only 52.9% had intervened. Changes were more likely to be adopted by women (p=0.025). Among the preventive activities, asking for help (12.1%) and using walking aids (18.5%) were the least adopted. Fall prevention programs that focus on modifiable risk factors are needed.]

[The Manifestations of Headaches and their Impact on Everyday Activities]

[In Hungary at least 3.5 million people had headache. The aim of our research was to acess the occurrence of headaches, causes and symptoms. Quantitative, cross-sectional research was carried between 01.03.2021 and 30.11.2021. The target group is university students aged 18-45 years who have experienced headaches in the last 3 months (n=111). We used self-made and validated questionnaire. Descriptive statistic, two-sample t-test, correlation, ANOVA and Khi-square tests were calculated using SPSS 22.0 program (p<0.05). We detected: 63.06% tension, 8.11% migraines, 4.5% clusters, 24.32% mixed. The intensity of pain is significantly related to the frequency and duration of the appearance of headaches (p<0.05). Pain intensity doesn`t show a significant connection with stress levels, development of sleep disturbance, types of headaches, decreased concentration and learning methods (p>0.05). It can be said that the intensity of pain is influenced by the frequency of the appearance of headaches and the duration of pain.]

[Alaine Polcz, the Hungarian Pioneer of the Hospice Movement, was born 100 years ago]

[Dr. Alaine Polcz of the Hungarian pioneer of the hospice movement and thanatology was born 100 years ago, on October 7, 1922. She is hungarian psychologist, writer, wife of Miklós Mészöly of the Kossuth Prize-winning Hungarian writer. On the occasion of the centenary year, we present Alaine’s work and experts share their personal and professional thoughts about her. In 1991, Alaine established the Hungarian Hospice Foundation. It was then that the hospice movement began to develop, the aim of which is to provide help not only to the sick, but also to the family members who care for them during the period of illness and grief. Her writing legacy and wise insights have influenced many people to this day and are with us.]

[The Impact of the Coronavirus Epidemic on the Mental Health of Hungarian Nurses]

[The aim of our research was to find out the impact of the epidemic on the mental health of nurses in Hungary during the second wave. We assessed the levels of lethargy, anxiety, insomnia, irritability and insecurity before, during and, if there was a reassignment, during the epidemic and what influenced these symptoms the most. Descriptive statistics using SPSS 25.0 and Microsoft Excel 2010 were used along with χ2-test, ANOVA test, T-test (p<0.05). The results of 700 respondents showed that all the five symptoms mentioned above increased in value as a result of the epidemic (p<0.0001). Appropriate protective clothing decreased the value of anxiety (p=0.043). 11.6% of respondents indicated that they were not tested. Willingness to vaccinate was 70% at this time, significantly influenced by education (p=0.039). The coronavirus epidemic has significantly worsened the mental health of nurses. The difficulty of the period is also indicated by the fact that a large proportion of nurses were deprived of social contacts and 60% indicated that they had thought about leaving the health care system.]

[The role of anaerobic bacteria in brain abscesses: a literature review]

[Brain abscesses are potentially serious, life-threatening diseases that pose a complex diagnostic challenge not only to neurosurgeons but also to clinical microbiologists, neurologists, psychiatrists, infectologists. The etiology of brain abscess is usually polymicrobial, most commonly involving a variety of aerobic and obligate anaerobic bacteria. Epidemiological studies on the anaerobic etiology of brain abscesses are common between the time period of 1960s and 1980s, but today there are very few new publications on the subject. The role of anaerobic bacteria in this disease was presumably underdiagnosed for a very long time, as many laboratories did not have the adequate laboratory capabilities for their cultivation and identification. The purpose of this review is to summarize the available literature on the etiology of obligate anaerobic bacteria in brain abscesses, including their prevalence and current therapeutic recommendations.]

[The Multiple Sclerosis Registry of Szeged]

[Multiple sclerosis (MS) is a rare disease of the central nervous system considering the total population, the prevalence in Hungary is 83.9/100.000. The first MS registry was established in Denmark in the middle of the 1950’s. This was followed by the establishment of several national, then international databases with the number of enrolled patients in the hundred-thousands. At the beginning, the primary goal of the registries were the epidemiological surveys, focusing on the number of patients, the prevalence, the incidence, the mortality and the co-morbidity. As of today, however, with the rapid advancement and development of new disease modifying therapies (DMT) with different effectiveness and adverse reactions, the therapeutic use of the registries became even more essential: the modern, up-to-date, well established registries become integral part of the DMTs’ monitorization. The Multiple Sclerosis Registry of Szeged was first established as a “paper-based” database, then, in 2012, it was upgraded to an electronic, easily contactable and useable internet-based registry. As of today, it contains the socio-demographic and clinical data of more than 600 patients; we constantly add new patients as well as keep the registry up-to-date with the refreshment of old patients’ data. Aside from the “classical” clinical data, it can be used for the recording and assessment of the MRI scans and the data on psychopathological and quality of life assessments, which are becoming more and more important in everyday MS management. The establishment of the internet-based registry incredibly helped both the monitorization of the effectiveness of DMTs, and the success of the new epidemiological and psychopathological surveys. ]

[The history of acute stroke care in Hungary ]

[Stroke is one of the most frequent causes of death and the most important cause of permanent disability worldwide and also in Hungary. The Hungarian medical literature has mentioned this disease and has been giving recommendations for its treatment since 1690. Initially folk medicines, herbal preparations and phlebotomy were used as standard therapy. Later, cooling the head joined these methods. Pharmacy preparations emerged at the middle of the 19th century. From the middle of the 20th century, products of the pharmaceutical industry like blood flow enhancers and neuroprotective drugs were in the frontline of the acute care. Anti­hy­per­ten­sive, antithrombotic and lipid-lowering medications became part of stro­ke prevention. Imaging techniques – mainly computer tomography of the brain and ultrasound examination of the cervical large arteries – have radically changed the diagnostics of cerebrovascular diseases from the middle of the 1980s. Since the 1990s, diagnostic and therapeutic decisions are based on reliable evidence from good quality clinical trials. Since the beginning of the 21st century, reperfusion treatments (intravenous thrombolysis and mechanical thrombectomy) re­present the most effective emergency care. The current direction is the extension of the therapeutic time-window of reperfusion treatments based on sophisticated neruoimaging. This review provides a brief summary of the development of stroke care in the last three and a half centuries as reflected in the Hungarian medical literature. ]

[Epidemiology, cost and economic impact of cerebral palsy in Hungary]

[Objective - The purpose of our communication was to determine the total cost of cerebral paretic patients in Hungary between 0 and 18 years and to assess their impact on the national budget. Methods - Based on the data of Borsod county we calculated the CP characteristics. The cost of CP was determined by routine care of individuals. Lost Parental Income and Tax were calculated on the basis of average earnings. The ratio of GDP, Health and Social Budget and Health Budget to CP is based on CP annual average cost and frequency. We have developed a repeatable computational model. Results - Of the risk groups, premature birth (30.97%), low birth weight (29.64%), perinatal asphyxia (19.47%) were the most common. Source is unknown of 37.61% of the cases. CP prevalence was 2.1‰. The two-sided (59.7%) and the one-sided (19.0%) spastic pareses dominated. The most serious form is the two-sided spastic paresis (42.5% GMFCS 3-5 degrees). Epilepsy was 22.0%, incontinence was 27%, mental involvement was 46%. Care for one child up to 18 years of age costs an average of 73 million HUF (€ 251,724). The lost family income was 27.36 million HUF (€ 94,345), and lost tax and health care contributions were 14.46 million HUF (€ 49,862). Additionally, 0.525% of the GDP, 0.88% of the full health and social budget and 1.83% of direct medical costs were spent for CP families. Conclusion - The cost of CP disease is significant. Costs can be reduced by improving primary prevention. From the perspective of the family and government, it is better to care for families so they can take care of their disabled children.]

[The epidemiology, classification of hypertension and the Importance of risk stratification]

[Hypertension, which is the most important independent risk factor of cardiovascular mortality, affects approximately 3.5 million people in Hungary. Among them the rate of the unknown, known, but untreated or uncontrolled hypertensive patients is more, than 50%, a fact, which underlines the importance of screening and proper treatment. Since 2017, there is a difference in the classification of hypertension between Europe and the United States, based mostly on SPRINT study, which was pub­lished in 2015. However, in the treatment recommendations more similarities are present. Risk stratification must be the part of hypertension care. In risk communica­tion as an alternative, the concept of vascular age was introduced, which mirrors more suggestively the patients' vascular hazard. In this review the author overviews the novelties in hypertension epidemiology, classification, and risk stratification.]
