[Multimorbidity among older people participating in geriatric exercise programme]

[Objective: To determine the exercises and dosis of the effective and safe geriatric exercise programme it is essential to know that what kind of chronic diseases an elderly people suffer from. However, the old age is characterized by multimorbidity which means that the same person may have two or more chronic diseases at the same time. Methods: The aim of our study was to investigate the prevalence of chronic diseases associated with aged and multimorbidity among older people participating in geriatric exercise programme. Data were collected with self-administered questionnaire at sites of geriatric exercises programmes, Budapest. Results: Seventy one percent of the sample of 180 participantes (N=128) suffered from more than one chronic diseases. In our sample, the proportion of people with osteoporosis and obesity was significantly higher than the national average. At the same time, the prevalence of type 2 diabetes, lower limb joint disease, cerebrovascular accident and myocardial infarction was significantly lower than the national average. As regards to asthma and hypertension the proportions observed in our sample did not differ from the national average. Conclusions: In case of newcommer older people we need to pay more attention to the effects of chronic diseases associated with aging and of multimorbidity.]

Use of Imogene King’s Nursing Model in the Care for a Patient with Heart Arrhythmias


Objectives: To discover the benefits of implementation of Imogene King’s nursing model into nursing care in a patient with arrhythmias. To find out how these patients are limited in saturation of their own needs and what are the most severe limitations for patients with arrhythmias. Material and methods: Qualitative research using semi-structured interviews. The interview with patients with arrhythmia was conducted in accordance with the structure of King’s nursing model. Results: The research group consisted of 20 respondents; 13 men and seven women. All respondents expressed satisfaction with their cooperation with staff. The respondents described a change in the subjectively perceived state, the psychic state, and emotions. Conclusions: Assessment of the subjectively perceived quality of life of patients with arrhythmias using the King’s interaction model allows nurses to see the patient from the holistic point of view and plan and provide individualized care.

[The challenges of education in gestational diabetes ]

[The aim of the author is to draw attention to difficulties of pregnant women’s education who suffer from gestational diabetes. Due to the global increase of gestational diabetes, this topic’s importance and actuality is getting more relevant. The key of the successful diabetes treatment is the education. In the course of that it is important to choose a strategy, with that the patient can get as much special skills as she can and will be able to keep the ideal and progressive balance of metabolism. Through to the personal education here is a chance to put an end to the patient’s fears, to increase their motivation and to encourage them. ]

Physical activity as primary prevention of metabolic syndrome


Metabolic syndrome is constantly discussed, together with cancer diseases, as one of the biggest threats to the 21st century. Despite the differing indicators of specific diseases behind the metabolic syndrome, it is to be understood as a very risky aspect of health. Primary prevention through life style modifications, specifically reduction of the sedentary way of life and integration of regular physical activity into daily life of children, adults and seniors is an appropriate tool of prevention of metabolic syndrome. A number of valid studies show that increasing physical motion contributes to improvement of diseases that stand behind the metabolic syndrome. However, healthy adult population of the Czech Republic shows distinctive dislike of physical activity and primary prevention is insufficiently supported both by experts dealing with this issue and at political level while secondary prevention prevails. Therefore we consider it imperative to involve more funding into programs supporting physical activity. It is also necessary to explore forms of education within the physically active life style.

[This review summarize the decreased physical activity and physical function of chronic kidney disease patients from the early stage of their renal disease; the favourable effects of exercise training on physical activity and function as well as on progression of chronic renal diseases. At the end, there is a recommendation for implementation of exercise in this renal patient population. [Type 2 diabetes mellitus and metabolic syndrome are important risk factors of cardiovascular morbidity and mortality. Both diseases present with a similar set of metabolic disturbances including hyperglycaemia, hyperlipidaemia, hypertension and obesity. Therefore, their nonpharmacological treatment is based on similar principles. Medical nutritional therapy aims to promote moderate weight loss through decreased energy intake, and to correct metabolic disturbances by ensuring appropriate composition of micro- and macronutrients. In a healthy diet, carbohydrates and cis-fatty acids make up approximately 60 to 70% of total energy intake. It is important to reduce the intake of saturated fatty acids and trans-fatty acids. The consumption of foods with low glycaemic index may be beneficial for diabetic patients. The increase of physical activity (both aerob and resistance exercise) is useful in maintaining weight loss, and it also improves blood lipid levels and blood pressure. Abandonment of smoking results in significant cardiovascular risk reduction. Lifestyle changes should include all of the above factors in order to achieve most reduction in morbidity and mortality associated with type 2 diabetes mellitus and the metabolic syndrome.]


[Mechanotransduction, or the impact of physical activity on bone architecture]


[It has long been known that, along with bone mineral content, bone strength is also fundamentally determined by its architecture.This architecture is shaped primarily by the forces that act on the bone, i.e., gravity and muscle traction conveyed by the tendons.Thus the bone acts as a kind of a mechanostat. The authors provide an overview of the literature on the systems that regulate mechanotransduction turning mechanical strain into bone texture. Regularly performed movements that provide a frequently changing axial load induce an extracellular fluid flow in the lacunar system of the bones.This flow induces prostaglandin synthesis in the osteocytes, which in turn inhibits the Receptor Activator of Nuclear factor κB (RANK) - RANK-Ligand (RANKL) mechanism through the secretion of osteoprotegerin by osteoblasts.This leads to osteoclast inhibition. Furthermore, leptin secretion by osteoblasts increases, which enhances osteoblast activation and inhibits the apoptosis of osteocytes and osteoblasts by both an autocrine and paracrine route. All these together act in the direction of bone formation. Based on the available evidence, the authors conclude that regular exercise results in an increased bone mass, better muscle strength and firmer balance, which leads to a decreased fracture risk.Thus, physical activity, through its beneficial effects on cardiac and bone health described above, contribute to the improvement of the quality of life.]

[The role of a weekly geriatric exercise programme in successful aging]

[Objective: The aim of this retrospective study was to examine the effects of a weekly held group multicomponent exercise programme (consisting of aerobic, strengthening, flexibiliy, and balance exercises) on the functional abilities (muscle strength, walking speed, and static balance) among elderly people. Methods: Thirty eight older people were divided into two groups: the active group for those elderly who took part is the training for at least 2 years, and the inactive group for those who did not take part in the training before. The global muscle strength in the lower extremity was measured with the 5 sit-to-tand test, the walking speed was measured with the 4 meters walking test. To examine the static balance, we used the one-leg stance test. To determine the subjective well-being, a Visual Analog Scale was used. Results: The Active group was significantly better in 5 sit-to-tand test (t(36)=2,602; p=0,013; Cohen’d=0,99), and marginally significant difference was found in the 4 meters walking test (t(36)= 1,769; p=0,085; Cohen’d=0,66) to the benefit of the Active group. In the term of static balance, we could not find significant difference. Conclusions: This programme for elderly people is effective to improve the global lower limb muscle strenght and walking speed of the elderly, but not very effective in improving static balance.]
