[Computed tomographic examination of cranial lesions, a paleoradiological approach]

[Background and purpose - Introducing the multidisciplinary paleoradiology research at the Institute of Diagnostic Imaging and Radiation Oncology of the Kaposvár University, highlighting the cases with potential central nervous system involvement - from the scanning methods to the 3D printing - in order to draw attention to the historical background and clinical aspects of certain pathological conditions. Methods - The authors developed the examination protocols for three different CT scanners. Among the examined archaeological remains cranial lesions were identified in 26 cases, from which 4 cases with potential central nervous system involvement are demonstrated. The scanning parameters and the advantages of secondary image reconstructions (multiplanar reconstruction, maximum intensity projection, three-dimensional volume rendering technique) are presented with the cases. Results - The authors demonstrate a case with destructive skull lesions due to syphilis from the 15th century AD, a condition rarely seen or even unknown nowadays in the modern world. With the CT images of the skull base fracture from the Iron Age, signs of healing could be verified. Using the CT images a non-invasive approach is presented in the case of the craniofacial osteosarcoma in order to visualize the local status and the direct intracranial propagation. Advantages of the 3D VRT reconstructions are shown in the case of unilateral coronal suture synostosis. Conclusion - Paleoradiological CT examinations serve as a non-invasive, non-destructive tool for studying archaeological remains and artifacts. The special applications provided by the imaging modality contribute to the conventional paleopathological investigations. Title of the article: Bilateral acute angle-closure glaucoma induced by duloxetine

Introduction - To present a rare case of bilateral acute angle-closure glaucoma secondary to duloxetine administered for the treatment of depression. Case presentation - A 46 year old woman developed bilateral closed angle glaucoma after 15 days of duloxetine usage. Intraocular pressures (IOP) were 52 and 55 mm Hg in right and left eyes respectively, with shallow anterior chamber and angle closure on gonioscopy. Discontinuing duloxetine treatment, initiation of antiglaucomatous treatment and bilateral Nd:YAG Laser iridotomy obtained normalized IOP and anterior chamber depth. Conclusion - Duloxetine, used in the treatment of depression can be responsible for acute angle-closure glaucoma by leading to mydriasis and ciliary effusion.

[Retinal ganglion cell layer and visual function in patients with progressive external ophthalmoplegia caused by common mtDNA deletion]

[Aim - Mitochondrial (mt) disorders are metabolic conditions with multiorgan involvement, which often cause neuroophtalmological symptoms. The aim of the study was to investigate the relation between progressive external ophthalmoplegia (PEO), visual pathway and mitochondrial DNA (mtDNA) mutations in patients younger than 55 years of age. Methods - Five female patients (35 to 53 years of age) with mithochondrial disease were investigated. Automated threshold perimetry (Octopus G2 test), scanning laser polarimetry (GDx-VCC and GDx-ECC) and Fourier-domain optical coherence tomography (RTVue-100 OCT) were used in addition to detailed ophthalmological examination and evaluation of visually evoked potentials (VEP). Frequent mutations of the mtDNA were investigated in the patients’ blood and muscle samples. Results - PEO of various severity levels was found in all patients, using clinical tests. Genetic testing showed “common deletion” of mtDNA in all cases. For both eyes of 4 patients functional and structural ophthalmic tests had normal results. In one patient decreased visual acuity, reduced retinal nerve fiber layer thickness and prolonged L3 VEP latency time were found without optic disc damage and visual field deterioration. Conclusion - In 4 of our 5 patients with PEO due to common deletion of mtDNA retinal ganglion cells and visual function remained normal for a long period of life.]

[The apparatus which controls our kidney too. - Part 1]


[The series gives a brief overview on the discovery of the juxtaglomerular apparatus (JGA), an interesting story, as well as on details of its structure and function down to the molecular level. The discovery of JGA, i.e., a phylogenetically ancient organ, is a fine example of the close morphological and functional correlations characteristic of living organisms. Presented are the JGA related misconcepts and the underlying theoretical and practical difficulties. Utilization of the most modern methods, such as atomic force microscopy, as well as the in vivo multiphoton laser microscopy revealed previously unrecognized phenomen highlighting the ambiguities of textbook information, accepted paradigms. The author is looking for relationship between the new and provocative theoretical research and clinical consequences of pharmacological interventions. He shows that JGA is not only a participant of the salt-water balance and blood pressure regulation, but it can also play a significant role in the pathogenesis of the major public diseases. Finally, he makes an attempt to analyze the current research directions that predict some potential scientific discoveries and describe some general lessons from his own research career.]

[Postconditioning in major vascular operations for the prevention of postoperative renal complications]


[Objectives: During vascular surgeries on the abdominal aorta, lower extremities suffer ischaemia-reperfusion (IR) injury which can lead to rhabdomyolysis. A severe complication is the myonephropathic metabolic syndrome with acute renal failure. The aim of the study was to investigate whether postconditioning (rapid repetitive cycles of ischaemia and reperfusion on the onset of the organ reperfusion, a novel technique to reduce ischaemia-reperfusion injuries) could prevent renal failure in major vascular surgery. Subjects and methods: Male Wistar-rats underwent 180 minutes of bilateral lower limb ischaemia and four hours of reperfusion. Postconditioning consisted of 6 cycles of 10-second aortic occlusion/10-second declamping. Microcirculation of the kidney was detected with laser Doppler flowmeter. After 4, 24, 72 hours of reperfusion serum, urine, and histological samples were collected. Acid-base state was evaluated immediately after reperfusion. Results: After four hours of reperfusion there were no significant histological alterations in the muscle in contrast to the 24 hour rhabdomyolysis with inflammation. CK, LDH, AST levels increased in the acute phase but improved in the 24th and 72nd postoperative hours. Kidney histology and laboratory tests showed definite signs of acute tubular injury in control animals. In the early stage serum creatinine; seBUN/creatinin; FENa showed significantly (p<0.05) lower kidney injury in the postconditioned group. Postconditioning improved the kidney cortex microcirculation. Conclusion: Postconditioning can reduce the prevalence and consequences of renal failure after experimental major vascular surgery in rats.]

[Functional measurements of the hand’s circulation in Raynaud’s patients]

[INTRODUCTION - In patients presenting with isolated tissue perfusion disturbance without large vessel involvement the examination of hand microcirculation is of major importance. In our study we present the results of three examination methods used for hand perfusion monitoring which measure the tissue microcirculation in different depths. PATIENTS AND METHODS - We examined 58 primary Raynaud's patients using capillary microscopy, laser doppler perfusion imaging and hand perfusion scintigraphy with Tc-99m-DTPA. From our patients 38 were smokers, 42 patients frequently suffered from headache. For the validation of our laser Doppler results we involved into the study also 16 non-smoker healthy volunteers. For the standardisation of the results the studies were performed in a climatised room. During the laser examination we performed and analysed also the post-occlusion hyperaemia test. RESULTS - In the majority of our patients no morphological alterations were found with capillary microscopy. In primary Raynaud’s patients the perfusion values measured with laser-Doppler scanner in basic condition were significantly lower both in fingers’ and palm’s regions compared to healthy controls. In the primary Raynaud’s group the smokers had significantly lower hyperaemic response than the non-smokers and we measured also significantly lower FPR (finger-to-palm ratio) values with hand perfusion scintigraphy. Both with laser doppler imaging and hand perfusion scintigraphy there was no difference between the values measured in patients with or without headache. CONCLUSIONS - We consider of benefit the functional examination of hand circulation in all diseases involving the hand microcirculation.]

[Percutaneous coronary intervention in ischaemic heart disease]

[In ischaemic heart disease, if the medically treated patient’s anginal complaints and/or ischaemic symptoms are persistent, coronary angiography, and according to its results, coronary intervention (surgery or dilatation) may become necessary. The intervention is required in critical stenosis (>70% diameter) of the main vessels, the emergency depends on the clinical situation. Basic method of coronary angioplasty is the balloon dilatation, other tools (stent, rotablator, laser wire, atherectomy device, etc.) are also available. Periprocedural anticoagulant (heparin) and platelet aggregation inhibitor (aspirin, ticlopidine, GP IIb/III/a receptor blocker) treatment is required, the latter after the procedure as well. After stent implantation the lumen of the vessel is bigger and the incidence of major adverse cardiac events (acute myocardial infarction, repeated intervention, fatal outcome) is diminished. Decrease of serum lipid level improves the outcome of coronary angioplasty. It is applicable successfully for multiple lesions, occluded vessels, stable and unstable angina, in the early phase of myocardial infarction, in patients who underwent coronary surgery, and in old age too. Risk factors of the intervention are: tortuous vessel, significant calcification, stenosis in angle or ostium, luminal thrombus, urgent intervention, old age, female gender, congestive heart failure, unstable condition and acute myocardial infarction. When indicating the intervention, besides the probable results, it is necessary to consider the possibility of complications (myocardial infarction, malignant rhythm disorders, acute heart failure, bleeding, etc.). In left main stem stenosis, 3 vessel disease and in the case of 1 functioning coronary artery surgery would be preferable. With the present facilities the ratio of urgent surgical intervention as well as the mortality is below 1%.]

[The possibilities of CT and MR imaging in hydrocarbon research]

[INTRODUCTION - Cross-sectional medical imaging techniques were applied on geological samples (cores) in order to reveal their morphological and functional properties. Our aim was to reveal those CT and MRI methods that are capable to characterize certain petrophysical parameters and can be correlated with core morphology. MATERIALS AND METHODS - Similarly to pre- and postcontrast scanning in human diagnostic imaging CT scans of core samples were obtained in dry and flooded state, that helped to define and calculate morphological and functional parameters. Several MRI sequences were also tested, mainly spin-echos with short echo time. RESULTS - Several hundred meters of core samples have been examined in the last three years. Effective porosity can be calculated from CT data, that characterizes oil/gas storage capacity of the given geological formation. CT information is superior to conventional petrophysical methods due to its spatial resolution. Nevertheless, we can not quantify permeabilty yet, but describe it qualitatively. We found only limited use of medical MR imaging methods. DISCUSSION - Cross-sectional imaging, primarily CT scanning was introduced to the Hungarian oil and gas industry in the last three years. It seems that the method will be used more and more frequently through the entire process of oil/gas exploitation. Our further goals include the implementation of the CT measurements in other areas like environment control, and further develope measurement conditions.]

[Transmyocardial laser revascularization (TMLR) is a relatively new technique for the treatment of patients with angina refractory to other medical interventions, such as CABG or PTCA. The laser drills channels through the myocardial wall into the cavity of the ventricle. While the mechanism of action is still debated, clinical data show incentive results suggesting favorable outcome for patients. In the article, the published clinical and experimental data relevant to theoretical mechanisms and clinical results are reviewed.]

[Investigation of skin microcirculation in diabetes mellitus by laser Doppler flowmetry]


[The prognosis of patients with diabetes mellitus is mainly influenced by vascular complications which is partly due to the deterioration of the microcirculation.
