[Physical training in dialysis population]


[The impaired physical activity and the related increased cardiovascular risk is caracteristic in all stages of chronic kidney diseases. The regular physical activity has a beneficial effect on the metabolic risks associated with chronic kidney disease, dialysis and poor activity lifestyle, it also has favorable effect on the inflammatory state, poor physical performance, muscle loss and can improve the quality of life and life expectancy. Accordingly, the international and Hungarian guidelines suggest at least 150 minutes physical activity with moderate intensity per week - at least five days, 30 minutes each day. But there are no particular guidelines for dialysated patients. This article, without being exhaustive, in part using our own experiences, present suggestions for the physical activity of dialysated patients.]

[Pulse Wave Velocity measurement among university students ]

[The aim of the study: The purpose of our investigation is to prove the favorable effect of regular physical activity on arteries. Material and method: 42 students (29 women; 13 men; age 24 ±2,71 years) volunteered for our research. Anthropometric data, segmental-, visceral body composition (TanitaSBCA) and arterial stiffness parameters (Arteriograph) were measured. Participants took part in an 8-week training. Interventional-group: started training during our research, control-group: doing sports constantly for years. Results: Compared to the age decadal artertial pulse wave velocity (PWVao) reference value both groups showed significant difference. PWVao significantly improved because of training by the interventional group (6,23±0,79; 5,87±0,59 p<0,01); while in the case of the control group there was no significant change (5,85±0,54; 5,87±0,63 p=0,80). The body composition data showed minimal correlation with PWVao. Conculsion: Regular training has favorable effect on one stiffness parameter of arteries. Already 8 weeks enough to reach significant change to reduce arterial stiffness and this condition can be maintained with long-term regular training.]

[Physical inactivity and activity. Damage and benefit]


[Authors address the issue of inadequate physical activity worldwide and analyze their relationship with cardiovascular diseases and total mortality. In the inactivity "world map" it can be seen that in economically developed countries it is very common but other ethnic and regional factors also play a role and it is significantly more frequent at women. In our country this phenomenon due to combination of advanced civilization and computerization is also frequent. From 2000 to 2015, the trend of the presence of physical inactivity (FI) in the high income populations is steadily rising. Convincing evidence suggests that FI increases the risk of many common, serious diseases, including ischaemic heart disease, type 2 diabetes, breast cancer and colon carcinoma, and reduces the life expectancy. For these four diaseses, the incidence of FI occurs in about 1/3 of cases and 35% of total mortality. Its health value (PAF) ranges from 10 to 18%. Low (non-regular) exercise increases the risk of hypertension with or without family history. In subjects with moderate and intensive levels of physical activity (FA), there is a significantly lower mortality rate and the rate of occurrence of major cardiovasculars (CV) diseases compared to those with lower grade FA. Behind the positive effect of physical activity there is a multiple and complicated mechanism that manifests itself in the vascular system, in the physiological adaptation of the heart and in other metabolic and cellular effects. ]

[The effect of an alternative training method on the pelvic floor muscle]

[Aim: In our study we analysed an alternative training method alone or in combination with pelvic floor muscle (PFM) with deep abdominal muscle strengthen the PFM better. Material and Methods: The study was conducted on women (n=5, mean age ±SD: 51,8 years, ±9,23 years). The Callanetics® gymnastics consisted of training sessions of 2x1 hours (10 weeks only Callanetics® exercises, 10 weeks in combined with PFM contractions). The subjects completed a questionnaire (risk factors, symptoms) and to measure the body parts’ circumference at 0. and 20. weeks. The measurement of the conditional capacitance of the PFM was performed by EMG before the gymnastics, then at weeks 10 and 20. We used R Statistics Software. Results: Significant decreseing were observed in the circumference of extremities and on isometric contraction improves (p=0.036). The dynamic strength showed an increasing tendency. Conclusion: The alternative training method significantly increased the maximum contractions of the PFM.]

[Family medicine as a career. Medical students’ attitudes and vocational choice motivations]


[INTRODUCTION - The large number of vacant general practices is a burning issue in Hungary. The entering of new colleagues into the general practitioner speciality training does not pose a real solution to the human resources crisis in this field. Our aim is to assess medical students’ attitudes and knowledge about general practice. SAMPLE AND METHOD - Cross-sectional survey with self-completed questionnaires at the University of Szeged, with the participation of 94 fourth and fifth year medical students in 2016 and 78 first and fourth year medical students in 2017. RESULTS - In 2016 1% of students planned for sure, and 16% planned probably to work as a general practitioner in the future. In 2017 3.9% of first-year students planned definitely to be a general practitioner, and 15.4% planned that probably. Among fourth-year students 0% of students planned for sure, and 19.2% planned probably to work as a general practitioner in the future. Whatever the presence of family medicine in undergraduate training influenced the medical students’ opinion about the profession positively (0.4-1.3 on a scale based on the direction and strength of the effects of certain factors ranging from -5 to +5). Those who were interested in family medicine considered the situation of healthcare significantly worse (p=0.027), than those who were not interested. To make the profession more attractive the following factors may play the most important role: the more intense presentation (I: 37%) of general practice in undergraduate training, improving the prestige of family medicine (IV: 31%), high-quality work (IV: 39%). CONCLUSIONS - Few medical students plan to work as general practitioner in the future. The most effective way to raise interest in family medicine is to increase the students’ knowledge and awareness of this specialisation, and the more intensive presentation of family medicine in undergraduate training is a key issue.]

[Physical Dimensions of Quality of Life on Dialysis and Renal Transplant Patients]


[End stage renal failure is a disease with a serious impact on the everyday life of patients. The aim of this study is to measure and compare the quality of life of patients on hemodialysis and following renal transplant in Szeged, based primarily on physical dimensions and to compare the results with similar national and international data. Throughout the research the authors used the Kidney Disease Qualitiy of Life Questionnary- Short Form-36, the Illness Intrusiveness Rating Scale, and their own questionnary. They involved 111 patients into their study from September 2018 to May 2019. The results show that among the dimensions regarding physical health the pain subscale in the renal transplant cohort reached the highest scores: 76±26 (average±SD) vs. 55±33 in the dialysis cohort. (p<0,001). The result of physical role functioning subscale was the lowest, reaching 69±25 points vs. 50±30 in the dialysis cohort. (p<0001). Analyzing the correlation of quality of life and illness intrusiveness, they found that the higher the quality of life result, the lower is the illness intrusiveness result (r=-0,478). Due to the results of the present study, it can be stated, that the quality of life in the dialysis cohort is lower in Szeged. These results may contribute to a holistic view of care of our patients. ]

[Minority students in Hungarian medical training]

[General health of minority people is usually worse than that of their majority peers and they often expe rience discrimination in the healthcare system. According to international literature, physicians belonging to any minority group are more likely to care for other mi nority people, therefore they may play a key role in reducing healthcare inequities. Anonymous, online questionnaire was distributed to medical students of the four Hungarian universities with medical schools (response rate was 8.86%). In this paper, we analyze our collected data about perceived discrimination with descriptive statistical methods. Results of confirmative statistical analyses (statistical tests) were considered exploratory in nature. 29.6% of respondents self-identified as a member of any minority. 63.0% of minority students and 53.8% of women indicated that they realized discriminated or were harassed in the last 12 months, meanwhile, 37.8% of non-minority students and 31.9% of males have experienced discrimination. Dis­cri­mi­na­tion related to ethnic origin, sexual orientation and disability are regarded as the most widespread forms of discrimination according to our respondents. Students are most likely to say that there is no age related discrimination on the grounds of age – both being under 30 years old (12.0%) and being over 55 (8.6%). Being the member of any minority group seems to have no effect on student’s/ one’s opinion how widespread the forms of dis crimination are. Minority students are more comfortable to work with a member of another minority. However male students feel more uncomfortable to work with a member of sexual or gender minorities compared to female counterparts. Mi­no­rity students tend to be more critical to the universities’ efforts to enhance diversity. Minority students and females may play a key role in reducing discrimination in medical training and in the healthcare system and in providing high-quality care for individuals who belong to any minority. Although there are more females than males in medical training they still report higher occurrence of perceived discrimination. However, it is important to emphasize the low response rate in our study, which does not allow us to draw any general conclusions.]

[Nursing career of the high-school students]


[Aim of the research: The aim of the study is to analyze the trends of student orientation in the age and compulsory education process by age group. In our research, we would like to answer the question of how much „popularity” the health care profession as a professional area among students participating in Hungarian public education. If you feel this area of expertise close to you, what kind of vision you have. Research and sampling methods: A self-constructed questionnaire was administered during 24 Oct 2017 - 13 Jan 2018. In all counties of Hungary, the authors sent it to at least one of the training sites and filled up with me (N=329). These training sites are involved in healthcare training. Data were analysed with SPSS 15.0 for Windows statistical program. Chi square, Mann-Whitney tests were applied (p <0,05). Results: Out of the 159 people (48%), 170 (52%) in the family have healthcare workers. When performing the controls of the distributions, we can conclude that there is no significant difference in the distribution between the two groups, so there is the same number - that is, statistically significant difference - of the proportion of those willing to choose the health care path. In terms of career choice, 276 (86%) of respondents believe it is likely to be oriented towards the health care path, with 46 people (14%) planning their life is in another direction. Seven of its principal did not answer the question. By completing the Mann-Whitney test, we can conclude that there is no significant difference between the two groups in the medians, so there is no difference - that is, statistically significant difference - with respect to career choice as a nursing profession’s esteem. Of the 329 participants, 215 (65%) replied that they would like to work as a nurse, while 114 (35%) responded that they would not choose this job. Conclusion: Don’t influence significantly the career entrant if there is a healthcare worker in the family. Who stand in front of career choice don’t influence significantly the appreciation but is more motivated by sympathy and helplessness toward another person.]

[The effect of sensory deficit induced by anterior crutiate ligament injury on postural control]

[The balance parameters and the loading symmetry was assessed in a subject with anterior cruciate ligament deficiency. A 14 days intensive training was performed mainly with eyes closed in closed kinematic chain exercises, loading the affected limb. Stabilometry and the assessment of loading symmetry was performed. The sway path was calculated in two direction. The sway path comparisons did not revealed significant differences only a tendency of decrease in anteroposterior direction. Interestingly we found an increased loading of the affected limb after our intensive training, with a mismatch between the subjective perceived symmetry and the objective symmetry data. The changed symmetry perception might be explained with the change in proprioceptive information processing induced by the training. In the future further examinations with higher case number are necessary to support this result. ]
