[Survey of patient education among patients with hip arthroplasty]

[Aim of the study: To analyse the training level of patients underwent hip replacement therapy in different periods of their health care. Methods: Traumatology and orthopaedic departments, ambulances and musculoskeletal rehabi-litations took part in the research. A self-made questionnaire was distributed among doctors and health professionals who cured these patients directly. The received data was analysed with the IBM SPSS Statistics 20. program. ANOVA test, 2-sample T-test and Chi-squared test were applied to analyse the coherences. P<0,05 was defined as the value of the significance limit. Results: 86 health care professionals participated in the survey. The overwhelming majority of them (45%) reported that unstructured, postoperative patient education was characteristic. Their education was dominated by communication techniques (n=68), while among the instrumental methods the use of information leaflets was dominated (n=13). Their activity was limited by the time lag and work overload (76%). Analysing the training levels, it was found that information about lifestyle changes and post-operative mobilization was significantly underused. Conclusion: Significant insufficiency were found in the education of the patients in terms of organization, and in the teaching methods and topics which were further aggravated by the mentioned hindering factors. ]

[Efficiency and safety of biphasic aspart insulin therapy in clinical studies]

[Six-to-eight years after the diagnosis of type 2 diabetes the majority of patients require insulin treatment. Premixed insulin therapy provides an insulin profile that is closer to the physiological profile than that achieved by basal insulin supplementation and, in some cases, may serve as an alternative treatment in patients for whom intensified insulin therapy is unsuitable. However, if premixed human insulins are used, nocturnal hypoglycaemia occurs relatively frequently. Furthermore, patients must keep a lag-time of 30-45 minutes between the injection of insulin and eating. In contrast, if premixed insulin analogues are used, there is no need for such lag-time and both nocturnal and severe hypoglycaemia are less frequent than with human premixed therapy. The superiority of premixed insulin analogues compared with premixed human insulin therapy has been confirmed by a number of prospective, randomised controlled trials and retrospective analyses. The author summarises the results of these studies, emphasising the beneficial effects of premixed insulin analogues in the therapy of type 2 diabetes.]

[Current questions of quality assurance in diagnostic radiology in the light of a visit to England]


[Physical-technical aspects of quality assurance in diagnostic radiology, because of its dependence on technology are of an extraordinary importance. The intention of Hungary to join EU makes at least the decrease of our lag in this respect unavoidable. Ministerial order 31/2001 (X. 3.) EüM which already came into force requires quality assurance in diagnostic radiology explicitly. This paper starts with definition of basic concepts, then outlooks shortly the history and present international situation of quality assurance in diagnostic radiology. We review preliminaries and the present situation in Hungary, including results of the National Patient Dose Assessment Programme till now. We think that the most efficient help to the initial steps of quality assurance in diagnostic radiology in Hungary may be the appropriate adaptation of experiences of the leading countries. Therefore we review experiences of one of the authors gained during visiting three medical physics centres in England in details. The following topics are discussed: legal requirements, types and levels of measurements, organizational problems, practical evaluation of measurements (including criteria of discarding equipment), patient dosimetry, personal dosimetry, mammography research, instrumentation of the radiology departments, calibration of measuring devices, questions of the so-called type testing and radiation protection training of workers.]

[Melatonin, sleep and the circadian rhythm: theoretical considerations and their chronopharmacological applications ]


[The predictive homeostasis of living organisms is an anticipatory adaptation to rhythmical environmental changes. A good example for this is the circadian rhythm preparing the organism for the alternation of day and night. The circadian rhythm of melatonin production anticipates the timing and duration of nocturnal sleep of human subjects. It also induces a sleep-like stimulusprocessing mode of the brain and - in case of adequate environmental circumstances - a soporific and in part, a sleep-inducing effect. Specificities of melatonin effects on sleep are the reduction of slow-wave EEG activity, as well as the increase in seep EEG spindling and REM sleep time. Like other substances with hypnotic properties, melatonin decreases core body temperature, but has also a strong chronobiotic effect that is expressed as rapid and strong phase shifts of the circadian rhythm, depending on the time of day of melatonin administration. Because light acutely suppresses melatonin production, adequately timed light exposition, containing also low wavelength components, together with exogenous melatonin, could be useful in treating jet-lag syndrome and other circadian rhythm disorders.]
