[Osteoporotic patient’s use of prescription drugs - pilot study]


[BACKGROUND - In Hungary, the number of the highest mortality hip fractures is between 12 000-15 000 per year. The cost of treating hip fractures is several times higher than that of preventive medical therapy. Thus, the compliance of patients with osteoporosis is of great importance. METHODS - Using the informatical database of St. András Rheumatology Hospital at Héviz, we collected one year’s prescription drugs for osteoporosis and compared them with the number of drugs obtained by the patients, determined from National Health Insurance data. RESULTS - In general, the patients obtained 75% of prescription drugs. From the 4354 boxes of prescribed antiporotics, 3637 contained bisphosphonate (not considering vitamin D and calcium). Within this group, 88% of combination preparations were obtained, which is a greater ratio than that of non-combination bisphosphonates (84%). CONCLUSIONS - On the basis of our results, we posit that prescription of a combination preparation somewhat improves the patients’ compliance. The low concordance of vitamin D and calcium preparations is worrying.]

[Current treatment and rehabilitation of the hip fractures - The role of orthopedic surgeons in the investigation and treatment of osteoporosis]


[The treatment of proximal femoral fractures is one of the most common surgical indications in traumatology, on the other hand, it requires a significant proportion of health expenditures. The structure of our society is going to shift toward the older generation in the next decades, which increases the importance of the above factors. New implants and surgical techniques appear in order to make the treatment more effective, thus allowing the patient’s early rehabilitation. In this article, we summarise the diagnosis and treatment of hip fractures, the most serious consequence of osteoporosis, and describe the results of rehabilitation. We describe the role and possibilities of orthopedic surgeons in the investigation and treatment of patients with osteoporotic hip fractures as well as possibilities of tertiary prevention and suggest possibilities of cooperation among the co-professions dealing with osteoporosis in the treatment of this patient group.]

[Hip fractures in Hungary between 2001 and 2008 - Assessment of the beneficial effect of bisphosphonates on the risk of hip fractures on the basis of Hungarian data]

[The overall prevalence of osteoporosis in developed countries is estimated to be 9-15%. Mortality in the first year after the fracture is 15-20%, and half of the survivors remain partly or fully dependent on others’ support. Owing to the increasing life expectance and the diseases of civilisation, the incidence of osteoporotic fractures is expected to double in the next thirty years. The network of centers that has been developed since 1995 under the National Osteoporosis Program and the accreditation system of the National Osteoporosis Center provided up-to-date education of the physicians (densitometry assistants) who work within the network. The diagnostic restrictions followed by the reduction of support to 70% since 2006 fall resulted in a dramatic reduction in the number of treated patients.]

[Pictures to the past Methodological possibilities in the palaeoradiology]

[INTRODUCTION - The radiology as a method is useful not only in the field of the traditional medicine, but in the historic anthropology examining the ancient human remains, and in the palaeopathology examining the pathological changes of the ancient human remains. The aim of our study was the correction of the palaeopathological methods and the radiological diagnosis of the ancient pathological remains. MATERIAL AND METHODS - Approximately 25 specimens originating from the Avar-, the Hungarian Conquest Period and from the Arpadian Age with pathological changes were examined. Besides these 11 naturally mummified individuals were examined by radiological methods, too. The X-ray examinations were done at the Department of Radiology of the National Medical Centre, in Budapest. RESULTS - Vertebral changes, pulmonary tuberculosis and traumatic lesions were found on the mummified individuals. The most frequent changes of the bone alterations were traumas: fractures of the ulna, clavicule, ribs, and trephanation of the skull. Several degenerative changes of the vertebral columns and of the articulation surfaces were detected (osteophyta, spondylosis, arthrosis). The rheumatoid arthritis, Paget-disease, facies leprosa, and the traces of vertebral tuberculosis should be mentioned among the rare pathological disorders. CONCLUSIONS - Since there are no sufficient palaeoradiological methodological references, it was necessary to develop new methods for X-ray examinations.]

[Glycylcyclines are a new class of antimicrobial agents, of which tigecycline is the first to have reached the final stage of clinical trials. It is the 9-t-butylglycylamido derivative of minocycline. The addition of a side-chain into position 9 overcomes the problem of the development of resistance typical to tetracyclines. Tigecycline inhibits translation of bacterial proteins through its effect on the 30S ribosomal subunit. The efficacy of tigecycline is impressive against multiresistant Gram-positive bacteria, including methicillin- resistant Staphylococcus aureus, penicillin- resistant Streptococcus pneumoniae, and vancomycin-resistant enterococci, against most genera of Enterobacteriaceae, including extended- spectrum β-lactamase-producing strains, and against most strains of Bacteroides fragilis, as well as atypical pathogens. Clinical studies showed good results in parenteral treatment for complicated skin, soft tissue and intraabdominal infections. Tigecycline will be most useful as empirical monotherapy in infections with suspected polymicrobial etiology and/or multidrugresistant agents.]

[Disturbances of the bone metabolism in type 1 diabetic patients]


[AIMS - Because of contradictory data in literature our aim was to study bone metabolic disturbances and their correlates with anthropometric and metabolic parameters in type 1 diabetic patients (T1DM). Since quantitative bone ultrasound (QUS) measures bone qualities different from BMD, and it has only been scarcely investigated in T1DM, our aim was to describe covariates of QUS parameters. PATIENTS AND METHODS - Osteodensitometry was performed (lumbal spine, femur neck - DEXA; calcaneal ultrasound) on 115 T1DM patients (34 male, 81 female; mean age: 41.4±11 [± SD] yrs; BMI: 23.9±3.0 kg/m2; diabetes duration: 21.6±11.7 yrs; HbA1c: 8.1±1.3%). In addition anthropometric, blood pressure and laboratory parameters (HbA1c, lipids, renal function, fibrinogen, homocystein, PTH, TSH, β-CrossLaps, vitamine D3, osteocalcin, osteoprotegerin) were measured, data using a questionnaire were collected. RESULTS - The prevalence of osteoporosis was 9/112 (8%). A further 21/62 patients with osteopenia were found. Disturbances of bone metabolism have been more frequently proven on lumbal spine (p<0.001). Using multiple linear regression modelling, the independent covariates of osteopathy were systolic blood pressure, body weight, β-CrossLaps and cystatin C. The average broadband ultrasound attenuation (BUA) was 114.2±14.9 in males vs. 108.4±16.3 dB/MHz in females (p=0.07), the mean speed of sound (SOS) 1552±26 in males vs. 1559±32 m/s in females (p=0.32). SOS values in addition to bone density were associated with fracture risk. The independent covariates of BUA were body weight and height (R=0.473, p<0.001), and of SOS only fibrinogen (R=0.305, p=0.032). CONCLUSIONS - According to our results the prevalence of osteoporosis in acceptable controlled T1DM patients is relatively low. The more common metabolic calcipenic osteopathy show a correlation with body weight, markers of bone resorption and diabetic complications/co-morbidities (nephropathy, hypertension) being therefore not only an a priori consequence but also a complication of diabetes mellitus. Our data provide baseline data of QUS in type 1 diabetic patients. Because of the frequency of lower bone mineral content and their known high fracture risk bone metabolism screening of T1DM patients has to be considered.]

[Management of hip fractures: practice, results, and problems in the last decade]


[Among fractures attributable to osteoporosis, hip fractures are associated with the most severe consequences and the greatest costs. Surgical therapy for this condition is continuing to improve. Increasingly recent implants and techniques are being developed and introduced to successfully control postoperative adverse events. The authors briefly outline actual practice in Hungary and present some results of the follow-up of their patients in the last decade. These show that, in order to improve outcomes of hip fracture patients, all domains of management need to be addressed.]

[As many as 80% of patients with schizophrenia have serious problems with the compliance at some point during their course of their treatment. In most cases, patients are partially compliant, taking only a portion of their prescribed medication. Early warning signs of such partial compliance may confuse some clinicians with non-response to treatment and may result in switching these patients to alternative oral antipsychotic drugs. This review focuses on factors that can contribute to partial compliance such as poor insight, negative attitude or subjective response toward medication, cognitive dysfunction, treatment-related side effects, substance abuse and complicated treatment regimen. Partial compliance is among the most common causes of psychotic relapse and the need for rehospitalisation. The reduced incidence of adverse side effects such as extrapyramidal symptoms with atypical antipsychotic agents has the potential to improve compliance in the maintenance treatment of schizophrenia. Administration of a long-acting injectable antipsychotic provides confirmation of whether patients have taken their medication. Furthermore, it allows physicians to distinguish non-response and non-compliance. Strategies for managing partial compliance include the use of a long-acting injectable atypical antipsychotic, psychoeducation and cognitive-behavioural interventions.]

[National Fracture Prevention and Risk Reduction Programme]


[Prevention of fractures due to osteoporosis is of crucial importance given the high mortality associated with these fractures, especially with those of the hip and vertebrae. Patients with fractures often lose their independence or require institutionalization. Osteoporosis is a special disease in that even though its public health significance is well known only 10% of patients are medically treated.There are 900,000 patients with osteoporosis in Hungary, out of which about 130,000 to 150,000 are currently treated. In April 2003 a new fracture prevention programme was launched.The aim of the programme is to prevent subsequent fractures in patients with fractures of the distal radius, proximal humerus, hip and vertebrae by thorough examination, risk assessment and therapy if necessary in the osteoporosis centres and in the traumatological units associated with them.The National Fracture Prevention and Osteoporosis Risk Reduction Programme has been adapted from Glasgow, Scotland, and is the first of its kind that aims to cover a whole country.]

[Disturbance of bone development in experimental hepatic cirrhosis in growing rats]


[INTRODUCTION -The pathomechanism of hepatic osteopathy is not fully understood.We investigated how bone parameters change in growing rats with experimentally induced fatty liver, liver cirrhosis and hepatocellular carcinoma. METHODS - Liver disease was induced by administration of CCl4 and phenobarbital (PB) following a single injection of diethylnitrosamine (DEN) in 55 Fischer 344 rats.Animals were sacrificed and their femur removed at week 8 or 16. Bone mineral content (BMC), femoral length, cortical index (ratio of cortical thickness and total diameter at the diaphysis) and ultimate bending load (Fmax) of femora were determined. Results of animals treated with DEN+PB+CCl4 (group DPC, n=21) were compared to untreated animals (n=14) and to a second control group treated only with DEN+PB (group DP, n=20). RESULTS - Fatty liver and cirrhosis developed in each animal in the DPC group (n=21) at week 8 and in a subgroup of these animals (n=11) hepatocellular carcinoma also appeared by week 16. No changes in bone parameters were observed in this group at week 8, but lower BMD, femoral length, cortical index and Fmax values were found at week 16 compared to the untreated controls or to the DP group (p<0.05 for both). In the DP group no fatty liver or cirrhosis was observed at any time. Femoral length and Fmax values were higher in the DP group at week 8 compared to the untreated controls (p<0.05 for both).At week 16, however, no difference could be detected. CONCLUSION - Experimentally induced liver cirrhosis and hepatocellular carcinoma are associated with growth inhibition and reduced bone mineral content, cortical index and mechanical resistance in growing rats.]
