[What turns labour pain into suffering? The individual ways of coping]


[INTRODUCTION - Pain is not only a sensory experience, but includes emotional, motivational and cognitive dimensions, thus women’s perceptions of labour pain are largely influenced by individual attitudes. The distinction between pain and suffering sheds light on valuable information. Our aim was to study the relationship between women’s labour pain and their sense of suffering, and the effects of epidural analgesia on their judgements of pain, stress and satisfaction. SAMPLE AND METHOD - 342 women participated in our study. Their pain was assessed with the Numerical Rating Scale, sense of suffering with the shortened Perceived Stress Scale, which were followed by questions of epidural analgesia administration, its perceived effects and satisfaction. RESULTS - Women’s feelings of pain and sense of suffering show a strong positive correlation (R=0,283, p<0,001); but with the increase of these dimensions their satisfaction decreased (R=-0,162 and R= -0,444 respectively, p<0,001). The administration of epidural analgesia did not have a significant effect on their judgements of pain, stress and satisfaction. Perceived stress had the strongest determining influence on satisfaction (b=-0,534, p<0,001). CONCLUSIONS - According to our results on pain relief, we suggest the wider use of various non-pharmacological methods. Alleviation of maternal fear and suffering through non-judgemental, patient and supportive attitude of caregivers is of paramount importance.]

[How to break medical bad news? The importance of the course for breaking medical bad news and an evaluation of his curriculum]


[Breaking bad news to patients is a difficult task for health care professionals and there are many unsolved issues in this area. There are several ways of delivering bad news and the quality of the communication could affect the whole life of the patient. However the wrongly delivered bit of bad news could not only cause a crisis for the recipient but will create a stressful situation for the communicator. The course created at the University of Pécs, Faculty of Health Sciences is specified to the delivery of bad news with modern communication methods, which uses video feedback method to analyse and develop the participants’ communicational attitude, thus providing potent aid for the acquisition of the proper communicational techniques, from which both the patient and the care giver can profit.]

[Delivery of the bad news communication in the pediatrics]

[The aim of the study: Evaluation of health professionals’ attitude and the circumstances of delivering bad news in a pediatric inpatient institute. Materials and methods: Exploring, quantitative and cross-section study with the help of self-administered questionnaires among doctors and nurses (n=109). The electronic software used for analysis: SPSS 20.0. Descriptive statistics, chi-squared test and ANOVA-test were applied (p<0.05). Satisfaction with the delivery of bad news and the circumstances of communication were considered as dependent variables. Demographic data, as well as experience in communication trainings/ skills in delivering bad news were defined as independent variables. Results: Health professionals working in the studied institute cannot rely on any protocol regarding the delivery of bad news. However 65.1% of respondents feel the need for a guideline. 56% of subjects did not have any education on the communication of bad news. Emotional support of parents is achieved through information materials (24.8%) and psychological guidance (60.6%). Conclusions: The study shows that in pediatric institutes it would be necessary to work out a guideline for delivering bad news and to organize communicational trainings for those involved.]

[Prenatal care and the role of midwifery in Europe and in Hungary]

[Aim of the study: To provide a picture about the prenatal care practice and assess the attitude of obstetrics and midwives about the new prenatal care system in Hungary. To compare the Hungarian prenatal care system with that of other countries in the European Union (Finland, England). Procedure and Sample: We used self-developed questionarre with 50 midwives, 40 pregnants and interview with 5 midwives and 5 obstetricians. To compare the prenatal care systems in the EU we used the experienses of a study trip in Finland and informations from international databases. Results: 8% of the midwives work in prenatal care, 32% of participants are not interested in this field. In their view the new system is not elaborated yet, they need more professional survellance. Obstetricians acknowledge the competence of midwifery, however they suggest to gain more practical knowledge on the field. 75% of participating pregnant women would have chosen the prenatal care of obstetricians. The role of midwives in prenatal care is significant in Europe. The Hungarian system is unique, which is approached to the european practice by introduce the new prenatal care system. Discussion: The new Hungarian prenatal care system has several loops in its structure and effectiveness from the view of midwives and obstetricians.]

[A családorvosok alvási apnoéval kapcsolatos ismeretei és attitűdjei. Megvalósul-e az OSAS szűrése a járművezetők egészségi alkalmasságának vizsgálata során?]


[Objective - Obstructive sleep apnea syndrome (OSAS) without treatment can cause serious cardiovascular, cardiorespiratory, neurological and other complications. Family physicians have an important role in recognizing the disease. The aim of the study is to assess the knowledge and attitude of family physicians related to sleep apnea. Whether OSAS screening is realized during the general medical checkup for drivers. Methods - In the cross-sectional study we used a validated OSAKA questionnaire in mandatory continuous medical education courses, supplemented with four additional questions. Results - 116 family physicians and 103 family medicine residents filled out the questionnaire. Hungarian family physicians, especially male doctors lack the adequate knowledge of sleep apnea. The average score of female physicians was significantly higher than that of males (13.4±1.8 vs. 11.7±2.6, p=0.005). The more sprecializations the doctor has, the higher the score. Zero or one special examination holders reached 12.5±2.3 points, two special examination holders 12.7±2.2 points. three or four special examination holders reached 14.0±2.1 (p=0.05). Residents’ average score was 12.1±2.4 points, which is higher than that of family doctors (p=0.012). Female residents also had higher average points than male residents (12.6±2.0 vs. 11.3±2.7; p=0.008). The size, location and type of the practice or the doctor’s age did not show any statistically significant correlation with the number of points achieved. According to our regression analysis, corrected to variables in the model, we found correlation between gender and medical knowledge, but there was no correlation between age, number of specialities, body mass index and the theoretical knowledge of the doctors. In terms of attitude female GPs had higher average scores than male GPs (3.5±0.6 vs. 2.9±0.6, p<0.001). Despite the modification of the 13/1992 regulation only 39% of the practices carried out regularly the required OSAS screening as part of the medical examination for a driving licence. Conclusions - Despite the high prevalence and clinical importance of OSAS, GPs often do not recognize sleep apnea and they have difficulty in treating their patients for this problem.]

[Physician attitudes concerning complementary and alternative medicine]

[INTRODUCTION - A crucial challenge for XXI. century health care is the presence of complementary and alternative medicine (CAM). A marked question of professional discourse has been characterizing patient CAM use, yet the attitudes, knowledge, and opinions of physicians have received far less attention. METHODS - This qualitative study involves semi-structured interviews conducted with 45 physicians. The narratives were scrutinized by employing Interpretative Phenomenological Analysis (IPA) via At­las.ti software. RESULTS - The narratives were coded ac­cording to attitudes and opinions concerning CAM, and subsequently participants were placed into 3 main groups. The first group is distinguished by a total rejection of CAM (n=11), while at the other end of the continuum, participants integrated biomedicine with CAM therapies (n=8). Those participants in between the two poles (rejection and integration) were characterized as „acceptance with reservations” and signify a heterogeneous category regarding composition and attitudes. COMMENTS - Most participants in our sample belonged to the „acceptance with reservations” category, that is, physicians who do not employ CAM in their praxis, albeit support their patients should they decide to utilize a CAM modality. Our inquiry aims to serve as a precursor to a quantitative study concerning CAM use.]

[Defending the statins]

[Taking into account the frequent occurrence of coronary heart disease and the mortality related to this, it is obvious that the primary and secondary prevention of cardiovascular events is nowadays a major challenge in medicine. The lipid lowering therapy has become the basic element of the prevention and the statins are first-choice drugs in this field. Despite the great amount of the evidences coming from studies based on the principles of the evidence based medicine, the statins are frequently in the crossfire of undeserved offensives. The unexplainable reluctance to statins is frequent not only among the misled patients but among doctors too. The attitude of the latter to the treatment determines the patients’ compliance. The change of this attitude is the aim of this paper. This potential change and the continuous education of the patients are the most common possibilities for the improvement of the bad statin persistence and adherence. ]

[Young men’s knowledge about the human papillomavirus]

[Introduction - Nearly the fifth of more than 100 HPV serotypes affect the anogenital region causing genital warts, penile and anal cancer. The incidence of male pathologies is lower than that of females (genital warts, vaginal, vulvar, cervical and anal cancer), however they are equally important in epidemiological terms. Methods - The aim of our questionnaire-based cross-sectional study was to assess the HPV-related knowledge of young men in 19 randomly selected high schools in Budapest. The anonymous questionnaires contained 54 items about socio-demographic and lifestyle factors, HPV-related knowledge and the attitude toward HPV vaccination. Results - We collected 530 completed questionnaires (86.74% response rate). 35.3% of the students knew that HPV was an STD and 3.2% was aware of transmission via skin contact. The majority (52.5%) linked cervical cancer to the viral infection, 7.7% linked HPV to the genital warts of females and 8.3% to the genital warts of males, 9.8% to penile cancer and 4.2% to anal cancer. 44.7% of the young men would have their future children vaccinated, while 24.5% remained uncertain. Conclusions - The young men’s knowledge about HPV was poor and they underestimated the risk of infection. This emphasizes the importance of targeted health education in this population. However, the students were mostly in favour of the HPV vaccination.]

[Attitude towards death, psychosomatic and pszichological status of nurses working in hospice care and oncology unit]

[Aim of the research: The aim of the research was to examine the fear of death, the attitudes towards death, the way of processing death, related to the workplace and to assess the burn out and psychosomatic symptoms among oncology nurses and hospice care workers. Research and sampling methods: To execute our research self-developed surveys were distributed online and printed (N=116) in July-August 2015, completed with a control group, as well. For data analysis we used SPSS version 19.0 with the following statistical tests: Chi square test, independent two sample t-test, Spearman rank correlation and variance analysis (p<0,05). Results: Oncology nurses had slightly more positive attitudes towards death, with significant (p=0,034) proneness to concentrate on positive events too, in case of death of their patients, and tend to attend Bálint groups more often (p=0,017), than nurses working in other unists. In the latter group psychosomatic symptoms were more frequent (p=0,032). No significant differences (p=0,234) were found between the groups in cumulative scale of fear of death. Fear of death had great effect on appearance of psychosomatic symptoms (p=0,011) and the extent of burn out (p=0,001). Conclusions: There were no significant differences in fear of death between oncology nurses and hospise care workers.]

[MC world - a board game of medical communication]


[In 2015 in the Institute of Behavioral Sciences of Semmelweis University a board game named MC World (Medical Communi­ca­tion World) was designed to further student learning in the field of medical communication by gamifying the learning material. 29 Hungarian and 28 foreign students were participating in a study where the acceptance of the game and its attitude change effect regarding medical communication were tested. The participants evaluated statements in connection with the game on a 5 point Likert-type scale. The survey responses suggest that both the Hungarian and the foreign medical students considered the application of the game in education as a practical and successful initiation. It draws the attention to better utilization of gamification as an educational facilitator and as a positive opinion forming opportunity.]
