[Hypertension and atrial fibrillation. Part 2. Basic methods of screening atrial fibrillation]


[Early detection of PF, especially short-term “paroxysmal aritmia”, is very important primarily in older individuals (over 65 ys), especially those with heart disease, hypertension or diabetes. Two methods are known for the early detection of PF: In one (regular screening) making 12-lead ECG examination for individuals over the age of 65 at fix times. The other (opportunistic screening) means that every person over the age of 65, whenever he or she percieves any complains, the physician will experience the pulse of the patient and in case of arrhythmia the EKG is made.]

[Hypertension and atrial fibrillation. Part 1. - Epidemiological data. The pathomechanism of PF in hypertension]


[Atrial fibrillation (PF) is the most common arrhythmia on the basis of analyzes conducted in different regions of the world. The main reason for this is the aging of the population. High blood pressure is one of the major factors of mortality in both developed and poor economical countries. The combined presence of the two diseases presents many hazards to our body. The most significant of these is the development of thromboembolic stroke, which is constantly increasing with age. The author analyzes in detail the aetiology called atrial cardiomyopathy, behind which there are complex structural, architectural, contractile and electro-physiological changes and leads to clinical manifestation of PF.]

[Everyday practice of atrial fibrillation treatment]


[The clinical importance of atrial fibrillation - the most frequent arrhythmia - is derived from the fact that it means a 5-fold risk of stroke/systemic embolism which contributes to the increased cardiovascular morbidity/mortality. Long-term oral anticoagulant therapy is a cornerstone of stroke prevention in patients with atrial fibrillation. Until recently Vitamin-K antagonists were the only available therapeutic option but its everyday use has several limitations, eg. bleeding risk, narrow therapeutic range, drug and food interactions and the need of monthly INR-control. The advent of NOAC-s may prevent a lot of difficulties regarding VKA-treatment and lead to as efficacious as and safer therapy than VKAs. These benefits can help better adherence of patients to the anticoagulant therapy which is one of the most important element of more effective stroke prevention. NOACs can be used more safely both in real life and in special patient populations (eg. elderly, type 2 diabetes, chronic kidney injury) than VKAs so they can contribute to effective cardiovascular risk reduction.]

A rare aetiology of stroke; myxomatous aneurysm caused by atrial myxoma

Atrial myxoma is a rare cause of stroke. In this report we present the case of a 52-year-old female patient who went to hospital suffering from a headache. Her neurological examination was normal except for a positive Babinski sign on the left. In the superolateral of the right, a Sylvian fissure consistent with a thrombosed aneurysm was detected using computerised tomography (CT). Diffusion MRI showed an acute infarction on the right MCA area. Transthorasic Echocardiography and ECG were normal. A -16×4 mm-sized fusiform perpendicular aneurysm on the M2 segment Sylvian curve of right MCA and a -6×4 mm-sized dissecting aneurysm on P3 segment of the left posterior cerebral artery (PCA) were observed in cerebral angiography. Transesophageal echocardiography (TEE) demonsrated a large mass with a suspected size of 2×2×1.5 cm on the left atrium. The mass was resected and on the eighth day after the operation, she had a temporary vision loss and hyperintensity on the T1 sequence was interpreted as laminary necrosis suspected on Cranial MRI. In follow up, she was stable with 300mg acetylsalicylic acid treatment. The main treatment is surgical resection in stroke caused by atrial myxoma.

Alteration of mean platelet volume in the pathogenesis of acute ischemic stroke: cause or consequence?

Introduction - Platelets have a crucial role on vascular disease which are involved in pathogenesis of ischemic stroke. Platelet size is measured as mean platelet volume (MPV) and is a marker of platelet activity. Platelets contain more dense granules as the size increases and produce more serotonin and tromboglobulin (b-TG) than small platelets. In this study, the alteration of MPV values were investigated in patients with acute stroke, who had MPV values before stroke, during acute ischemic stroke and 7 days after the stroke. The relationship between this alteration and risk factors, etiology and localization of ischemic stroke were also investigated. Methods - Sixty-seven patients with clinically and radiologically established diagnoses of ischemic stroke were enrolled into the study and stroke etiology was classified by modified Trial of Org 10 172 in Acute Stroke Treatment (TOAST) classification and, modified Bamford classification was used for localization and stroke risk factors were also evaluated. The platelet counts and MPV values from patient files in patients who had values before stroke (at examination for another diseases), within 24 hours of symptom onset and after 7 further days were analysed. Results - MPV values increased after stroke (10.59±2.26) compared with acute stroke values (9.84±1.64) and the values before stroke (9.59±1.72) (p<0.0001); this alteration of MPV values occured 7 days after stroke (p<0.016). There was a positive correlation between age and MPV values during acute stroke (r=0.270; p<0.05). Patients with atrial fibrillation had higher alteration in the time of MPV compared with patients without atrial fibrillation (p>0.006). We assessed for gender, men (n=38) had a higher alteration in the time of MPV compared with women (n=29) (p=0.013). Conclusion - Although there was no alteration of platelet counts, MPV values were increased 7 days after stroke in patients with acute ischemic stroke.

[Examination of cardio-vascular risk factors in the priority of atrial fibrillation]


[Objectives: Stroke-prophylaxis is the biggest question regarding atrial fibrillation. Their aim is to examine the risk factors, successfulness of frequency control, and the effectiveness of anticoagulant therapy. Methods: This quantitative cross-sectional study used a non-randomized sampling method. Enrollment criteria: anticoa-gulant therapy, atrial fibrillation, age 20-90 years, NYHA stage II. Excursion criteria: patients having elective intervention that influences INR rates. Patient data was collected from the records of the University of Pécs, Cardiology Clinic between 01.01.2012-31.12. They used linear regression, T-test, χ2-test, variance analysis (ANOVA), SPSS 20.0. Results: Significant correlation between BMI values-, systolic blood pressure-, modified medication and its effect on frequency control-, correct anticoagulant therapy, INR parameters and the increased risk of thromboembolia in patients with atrial fibrillation. (p<0.05) Conclusion: Stratification of risk factors and individual optimisation of anticoagulant therapy is important for effective stroke prevention and increased survival rate.]

[Hypertension and atrial fibrillation]

[Hypertension is the most important independent risk factor of atrial fibrillation, the most common clinically significant arrhythmia. Increased atrial wall tension, myocardium remodelling, activation of renin-angiotensin-aldosterone system (RAAS) and fibrotic remodelling are possible hypertension induced mechanisms which can contribute to developing atrial fibrillation. Knowing the relation of hypertension to atrial fibrillation is crucial in the need for effective and up-to-date therapeutic strategies. RAAS inhibitors and beta blockers prevent atrial fibrillation not only by lowering blood pressure but partly due to the inhibition of the above mentioned mechanisms. Complications of hypertension, such as left ventricular hypertrophy and hypertensive cardiomyopathy are substantially limiting the number of drugs that can be used for rhythm control. In most cases in the presence of hypertension initiation of anticoagulation therapy is necessary too.]
