Lege Artis Medicinae - 2015;25(04-05)

[INTRODUCTION - The prevalence of overweightness and obesity among children is increasing in Hungary, while the inadequate or excessive intake of some nutrients is also representative. Keeping these inappropriate lifestyle habits in adulthood could significantly worsen the health status of the rising generation. METHOD - The sample is representative for the 4-10 year old children of Budapest and Kecskemét city. The dietary assessment was executed, using the internationally accepted 3 day dietary record method. Body mass index was also calculated from the measured body weight and height, creating BMI categories according to the Cole methodology. RESULTS - The ratio of overweightness and obesity resulted in 21% (19% for the 4-6 year old, 24% for the 7-10 year old), while 13% of them belonged to the underweight category. The nutritional problems, identified by adults, like the excessive fat intake, its inappropriate composition, the high sodium-, low calcium- and vitamin D intake already appears in the early stages of life. CONCLUSION - The results prove, that the education for healthy nutrition among children is in high priority, in which all stakeholders, like the parents, the governmental and non-governmental organisations, the educational institutes as well as the food industry plays an important role. ]

[Mechanism of action and role of cilostazol in treatment of peripheral arterial disease]

KOLTAI Katalin, BIRÓ Katalin, KOVÁCS Dávid, CSISZÁR Beáta, TÓTH Kálmán, KÉSMÁRKY Gábor

[Intermittent claudication is a typical symptom of lower extremity arterial disease. Cilostazol is a reversible, selective phosphodiesterase-3 inhibitor which has antiplatelet, antithrombotic and vasodilator effects. It is indicated to improve maximal and pain-free walking distance in patients with intermittent claudication in the absence of rest pain or peripheral tissue necrosis. It can be beneficial in diabetic patiens with intermittent claudication, as it has been proved to prevent the development of foot ulcers. In combination with acetyl-salicylic acid it may help maintain stent patency after endovascular intervention and stent implantation. Cilostazol is contraindicated in heart failure. With cilostazol, a clinically proven effective drug has become available in the treatment of intermittent claudication which could improve walking and life quality of patients.]

[Day-to-day blood pressure variability measured with home monitoring for the verification of succesful antihypertensive therapy]

KÉKES Ede, PAKSY András, KISS István

[The value of „day-to-day” blood pressure variability based on home blood pressure measurements for demonstrating of successful antihypertensive treatment. Authors analyzed the effect of antihypertensive therapy on the blood pressure variability by day-to-day method based on home pressure measurements in a large number of hypertensive patients. During the four months observation period the mean blood pressure values were significantly decreased and the parameters of blood pressure variability were decreased as well, they reached the level of normal variability. The nowadays expected „double target values” has given us hope that we can successfully prevent the organ damages due to high blood pressure. The effects of some „drug family” and the drug-combinations to the blood pressure variability were analyzed. The RAS inhibitors (ACEI and ARB) reduced significantly the blood pressure variability, the ARB drugs more effectively. The ACEI agents combined with calcium antagonists or third-generation beta-blockers increased the normalization of blood pressure variability. Such effects were not observed with the ARB agents. Authors suggested the use of their method in every day practice. The guidelines also recommend this method.]

[Bone metabolism and body mass index in postmenopausal women]

TÁRCZY Csaba,Toldy Erzsébet, SZERB János, VARGA László

[INTRODUCTION - In addition to several other causes constitutional factors play an important role in the development of osteoporosis.Various aspects of bone metabolism were examined to explain the differences in bone density between women with low and high body mass index (BMI). PATIENTS AND METHOD - One hundred and ninetytwo postmenopausal women were included in the study. Bone density was measured by forearm densitometry. To assess bone formation, serum osteocalcin levels were measured, while the rate of bone absorption was estimated from C-terminal telopeptide levels of collagen type I measured in urine and blood. RESULTS - The prevalence of osteoporosis was higher in women with low BMI than in those with normal or higher BMI. Bone metabolism - both formation and absorption - was increased in both groups, however, in women with low BMI this increase was more pronounced and bone metabolism tended to be shifted to absorption compared to patients with normal or higher BMI. CONCLUSION - Postmenopausal lean women have accelerated bone metabolism compared to obese women. This fact and the shift to absorption may be the main reasons for the higher frequency of osteoporosis found by densitometry in women with low BMI than in those with higher BMI.]

[The malnutrition and the possibilities of its therapy in childhood]

GELENCSÉR Erzsébet

[Malnutrition is a very important clinical condition in pediatrics. However, undernutrition as negative impact on length hospital stay, risk of infection and health care costs. Moerver, it also effects the cognitive development of children. STONG is an easy-to use screening tool that can be recorded in a few minutes, and it does not require special knowledge measurement or follow-up. In Kaposi Mór Hospital (Kaposvár) the risk of malnutrition was assessed by STRONG nutritonal risk screening tool in 31 patient of child-gastroenterology ambulatory (02.01.2014-30.06.2014). In case of malnutrition counselling and use of special formulas are essential besides evaluating tehe cause of malnutrition.]

[Contact between the polycystic ovary syndrome and the metabolic syndrome]

KISS Katalin, VÁRADYNÉ HORVÁTH Ágnes, PAKAI Annamária

[Aim of the study: The objective of my thesis was to examine the presence of metabolic syndrome with the disorder of glucose metabolism among patients diagnosed with polycystic ovary syndrome (PCOS). Sample and methods: 98 female patients diagnosed with PCOS, aged 18-45, took part in the retrospective quantitative study, anthropometric data and laboratory parameters relevant to metabolic profile were analysed. We applied descriptive statistics with SPSS 17 software, as well as Chi2-test and regression analysis (p <0.05) Results: 46% of the study population were found to be overweight. There was a significant correlation between BMI index and blood pressure and lipid profile (p<0.05). Metabolic syndrome was confirmed by more than 40% of the patients Conclusion: The PCOS is an endocrine disease, which affects multiple organ systems. The patients should be screened towards metabolic syndrome to achieve a correct diagnosis, regardless of the BMI values. The findings of our research and the confirmed correlations highlight the need for a development in care protocols.]

[Objective - To evaluate the efficacy and tolerability of levetiracetam in children with drug resistant epilepsy from a retrospective study. Methods - We report the result of a study of 85 pediatric patients (mean 10.5 years, range: 1-24) with refractory generalized and focal epilepsy, who received levetiracetam as add-on treatment. The average duration of epilepsy was eight years, and the patient were treated with mean of 6.0 antiepileptic drugs befor levetiracetam was introduced. Results - Ten patients (12%) became seizuresfree, three (3%) responded with seizure reduction of more than 90%, 32 (38%) responded with seizure reduction of more than 50% following introduction of levetiracetam. No response to levetiracetam was reported in 34% (n: 29). Positive psychotopic effect was observed in 26 patient (30%). Mild to moderate side effects were reported in 11 patients (13%), consisting most frequently general behavioral changes, agression, sleep disturbances, but they ceased after decreasing the dose of levetiracetam. Mental retardation was associated with poor response and associated with more side effects. Conclusion - Levetiracetam is well tolerated new antiepileptic drug that may effectively improve seizures controll as an addon drug in resistant epilepsy in childhood with good tolerability.]
