[Education and psychological support of parents in cases of postnatally detected Down syndrome]

[OBJECTIVE - Since the 60’s several publications dealt with the phenomenon how physicians inform parents of newborns about postnatal recognition of Down’s syndrome and the support they receive right after breaking the bad news. Howe - ver, the rest of these studies concentrated on surveying parental satisfaction, while relatively few international studies deal with the other side of the communicational situation, the opinion of the informer. Our study focused on the circumstances of parental information in Hungarian institutions of obstetrics in order to evaluate the possibilities for interventions. METHODS - The Down’s team operating at the University of Pécs Faculty of Health Sciences carried out a national survey in 2005 - an interview-based questionnaire filled by physicians of institutions of obstetrics - with the help of the National Register for Congenital Diseases of the National Centre for Epidemiology and Down’s Foun dation. RESULTS - The coverage of the survey reached 74%. Rest of the surveyed institutions did not have information protocol, however, 70% of them believes it would be necessary. Only 44% of the physicians received communication training and 81% of them believe they can manage communication, 33% have felt that the mother of a newborn with Down’s syndrome would expect special help that the institutions are unable to provide. CONCLUSION - There are serious problems with the circumstances of parental informing in Hungarian institutions of obstetrics. This situation would obviously require intervention. An aimed communicational training based on international experience and exploiting the openness of physicians, as well as the establishment of information protocol could be elements of such intervention.]

[Conversation with the relatives]

[In our strong opinion in hospitals the efficient conversation among staff and patient and/or relatives is important. We try to draw attention to the psychosocial aspects of pediatric intensive care and the complex and special case of relative-staff communication via the introduction of a clinical case. Various medical experts were involved in the treatment of the infant and all participants witnessed the 4 months long life and the death of the newborn. The case of the intubed patient was painful and stressful for the medical staff. With our case-report, we try to present the spectrum, which forms during the treatment of a sickly born infant and the changes of the relationship of the medical staff and relatives as well as the engrossment of their interpersonal relationship along with the center of their communication problems and solution-finding. Our paper is centered around the psychosocial aspect of the clinical case and we would like to call the attention on how the medical experts communicated with the parents of the infant. ]

[Visibility and quality of life: international literature review of the state of LMBTQ healers]

[BACKGROUND - Mental vulnerability is a well-known problem among healthcare professionals. In additional LGBT physicians’ discriminatory experiences and the exposure of minority stress appear to be a greater risk for health problems. METHODS - We collate researches in English by using search terms “LGBT doctors” “LGBT providers” “LGBT physicians”; “attitudes of nurses”; “attitudes of medical students”; “attitudes of doctors”; “LGBT practitioners”, “homophobia among doctors” „“homophobia among physicians”; “LGBT in medical curricula”, “sexual and gender minority”, “minority stress”. We reviewed findings highlighting the disclosure of orientation, experiences, mental and physical health of LGBT healthcare professionals, and physicians’, nurses’, medical students’ and patients’ attitudes towards LGBT students and physicians. RESULTS - At present conditions of LGBT individuals have gained international attention also in academic conversation. Based on the literature lack of safe, supporting environment seems to be the highest problem for LGBT physicians and medical students. Both in Canada and the USA several steps were made to improve attitudes of medical professionals, and decrease discrimination towards LGBT individuals in healthcare system such as creating more supportive environment by ‘Promoting pride’ and integrating LGBT-related content into medical universities’ curriculum. CONCLUSIONS - Our findings suggest that in Canada and the United States the rate of discrimination have decreased meanwhile transparency of LGBT professionals elevated as a result of antidiscrimination policies and the educational development.]

[Trends in nurses’ conversation-leading skills]

[Aim of the study: The authors, in the course of their survey, assessed the conversation-leading skills of nursing students. Their objective was to analyse the extent to which the students’ conversational skills fulfi l the criteria for client-oriented treatment. Methodology and sample: The sample for the cross-section survey, conducted using the triangulation method, was selected using random sampling from among third-year students at the Nursing Department of Faculty of Health Sciences and Social Studies at the University of Szeged. The sample consisted of 55 full-time and correspondence nursing students. For the data gathering they used Helembai’s Trap Test method. The data was analysed using a combination of qualitative and quantitative methods. Results: The results pointed to domination of the studied conversations by the nurses. In the course of the communication the nurses primarily attributed cognitive communications to themselves, and affective communications to the patients. The nurses correctly recognised the importance of the principles of client management, which is shown by their effort to achieve a cooperative conversation-leading style. Conclusions: Based on the results, the principle of client-orientation is not fully upheld in the interactions of everyday nursing practice, a role in which is played by the biomedical focus and task-centred, paternalist approach that can still be observed in nursing today.]
