[Thinking about the nursing competences]

[Nursing is a complex profession consisting of several vocational skills such as empathy, theoretical, clinical, ethical, and communication skills. Beside these, technical knowledge is also essential for a nurse. The conventional methods of knowledge transfer are less and less able to meet the changed requests of the labor market. As a result of that nurse education has adapted to these changes, therefore the aim of the recent education system is to bring the acquisition of skills and competencies into prominence by using simulations as a helpful teaching strategy. Although for nurses working bedside at present improving their adequate technical skills still needs to be solved. The author wants to draw attention to the importance of maintaining and improving the acquired nursing competences, and that unregulated nursing competences are also important to be revised.]

[The endometriosis in the focus]

[Aim of the research was to investigate the deeply infiltrating endometriosis disease perioperative changes impact on the quality of life and on the sexuality. Research and sampling methods: A retrospective, partly longitudinal research, following surgery document analysis (n = 61) and we made a processing of the individually structured questionnaire with standard elements. We used 15D to measure the quality of life and MFSQ standard questionnaire to measure the sexuality (n=44). We calculated relative and absolute frequency, mean, standard deviation, paired t-test, χ2- test, correlation and one way analysis of variance (p<0,05) with the help of the SPSS 22. program. Results: 73.8% of the patients had at least 3 organ with endometriosis involvement and 82,5% were removed by laparoscopic surgery. The length of stay showed an upward turn tendency in the context of the complications (p <0.001). The sample vitality and sexual relations 40,9% were characterized by no complaint and between sexual partners and satisfaction (r = 0.442, p = 0.003) and between orgasm and sexual partners (r = 0.518, p = <0.001) medium correlation were detected. Conclusions: The surgery was efficient in the treatment of the DIE, but there are no detectable correlation the positive perception of sexual satisfaction. ]

[Recognizing colorectal cancer, education of patients and preparing them for the stomal therapy]

[Aim of the research: The study investigated the circumstances of the diagnosis of the disorder. The assessment of information given to patients, communication of the staff and the preparation for the stomal therapy was also part of the study. Research and sampling methods: A self-constructed questionnaire was administered during October and December of 2015. The inventory was uploaded to self-help sites for colorectal cancer patients and questionnaires were also handed out on surgical wards (N=114). Data were analysed with SPSS 22.0. Chi square, Mann-Whitney and Kruskal-Wallis tests were applied (p<0.05). Results: Colorectal cancer screening has never been attended by 81.6% of the responders. Constipation was a more frequent warning sign by women (p=0.045). The patients who received guidance from both doctors and nurses about oncology treatment were more satisfied with the information compared to those who only communicated with one person (p<0.001). The patients who could contact a stomal therapy nurse on-demand were able to handle their stomas autonomously (p=0.035). Conclusions: Information provided by more staff members help to dissolve anxiety. Contacting stomal therapy nurses on-demand should be preferred. ]

[Risk factors for naevus pigmentosus]

[Naevi are the most frequent disorders of the skin, which usually do not cause a problem. It can occur though, that some of them develop into a malign tumour, called melanoma malignum. This process, called malignisation, has perceivable introductory symptoms; and if they are detected in time and the affected birthmark is removed expertly, it can save the patient’s life. The article describes these symptoms and provides guidelines to the removal of birthmarks.]

Alexithymia is associated with cognitive impairment in patients with Parkinson’s disease

Cognitive dysfunction (CD) is a common non-motor symptom of Parkinson’s disease (PD). Alexithy­mia is a still poorly understood neuropsychiatric feature of PD. Cognitive impairment (especially visuospatial dysfunction and executive dysfunction) and alexithymia share com­mon pathology of neuroanatomical structures. We hypo­thesized that there must be a correlation between CD and alexithymia levels considering this relationship of neuroanatomy. Objective – The aim of this study was to evaluate the association between alexithymia and neurocognitive function in patients with PD. Thirty-five patients with PD were included in this study. The Toronto Alexithymia Scale–20 (TAS-20), Geriatric Depression Inventory (GDI) and a detailed neuropsychological evaluation were performed. Higher TAS-20 scores were negatively correlated with Wechsler Adult Intelligence Scale (WAIS) similarities test score (r =-0.71, p value 0.02), clock drawing test (CDT) scores (r=-0.72, p=0.02) and verbal fluency (VF) (r=-0.77, p<0.01). Difficulty identifying feelings subscale score was negatively correlated with CDT scores (r=-0.74, p=0.02), VF scores (r=-0.66, p=0.04), visual memory immediate recall (r=-0.74, p=0.01). VF scores were also correlated with difficulty describing feelings (DDF) scores (r=-0.66, p=0.04). There was a reverse relationship bet­ween WAIS similarities and DDF scores (r=-0.70, p=0.02), and externally oriented-thinking (r=-0.77,p<0.01). Executive function Z score was correlated with the mean TAS-20 score (r=-62, p=0.03) and DDF subscale score (r=-0.70, p=0.01) Alexithymia was found to be associated with poorer performance on visuospatial and executive function test results. We also found that alexithymia was significantly correlated with depressive symptoms. Presence of alexithymia should therefore warn the clinicians for co-existing CD.

[Neurophobia]

[Neurophobia is the fear of neurological diseases. Its main symptom is that medical students and young doctors are not able to utilize their basic neurological knowledge at the bedside. According to statistics, every second student suffers from neurophobia. This attitude could explain why in the last two decades less and less young doctors wanted to become neurologist. Medical students complain that they receive no instructions, and are afraid of loosing their interest and of facing the failure of their competency. The hardship of neurology was explained by the insufficient knowledge of anatomy and the infrequent encounter with patients. Even general practitioners have anxiety about neurological patients. The loss of interest in neurosciences seems to associate with insensitivity of human-centered culture and corruption of empathic thinking. The burnout syndrome of medical doctors and students can be explained by stress, loss of respect, permanent competition, independency that interferes with responsibility, stiff hierarchy of medical society, fear of diagnostic failures and of economical difficulties. The scores of depression in female students were higher than in male. The idea of the “good neurologist” has been changed. The business oriented care, the shortage of time, and the financial restrictions corroded the conventional practice and ceased the vocational idealism. At present, personal teaching is going to transform into impersonal multimedia learning. Because of the drastic change of values, the age of inner-oriented professionals has terminated also in the medicine. Medical doctors follow even less the traditional troll of professional behavior, but according the social demands, they choose their specialization for subsistence. The highly esteemed social status of neurologists and psychiatrists is going to sink in Europe. To reduce neurophobia it would be desirable 1. to introduce neurology training in the early years of medical school; 2. to teach neurology in all semesters, 3. to assure the effective teaching of neuro-anatomy and physiology, 4. to organize more one-to-one teacher-student communication. In the United States, residents participate in teaching during their residency training. To master neurology dedicated teachers are necessary whom neurology residents ought to meet personally with optimal frequency. However, these requirements seem to fail because of the chiefly technical characters of the actual reforms.]

[The role of nurses in care of patients with Guillain- Barré syndrome]

[The aim of study: development of nursing diagnoses that contribute to these patients’ quality of supply, and to work out a procedure for patients with Guillot-Barré syndrome, used by datas in the literature. Test method and sample: The test method is document analysis. I analyze and compare the literature da-tas, between 1 July 2007 and 31 October 2015, details of handled GBS patients suffering from neurologi-cal department. Following the presentation of GBS epidemiology, and anatomy background I present cli-nical symptoms of the disease, their justification, the opportunities to diagnosys, the treatment criterias, required in order to achieve quality patient care. Results: Treatment and Care of determining the recovery and rehabilitation of Guillain-Barré syndrome is concerned. The quality of care plays a key role in the patient’s quality of life regarding the future, which are summarized in a procedural order.]

[The trials and tribulations of district nurses past and present]

[The author wishes to highlight, for healthcare management, the work of paramedical professionals active in this field. She draws attention to the still largely untapped reserves of specialist knowledge possessed by these health workers. She suggests a method by which the currently underrated field of nursing could receive greater recognition, both in terms of working conditions and professional status. In this way nurses could more effectively provide a higher standard of care to patients registered with their practice, in the case of both acute and chronic diseases. A further aim of the paper is to present the structure and operation of the nursing practices that she envisions.]

[Care of patients with Amyotrophic Lateral Sclerosis]

[Aim of the research: Assessment of workers’ knowledge regarding patients suffering from a rare neurological disorder, Amyotrophic Lateral Sclerosis. Research and sampling methods: The target groups were: intensive care unit (Group I) and home care service nurses (Group II). The authors chose a self-developed questionnaire study, what was filled out on www.online.kerdoiv.com by 161 nurses. Results: Considering graduation from Group I. 17 (20%), from Group II, 27 (36%) have diploma in nursing education. Based on the answers, the ratio of the results are almost identical. Over 40% declare that they do not have sufficient knowledge, nurses can professional protocols in practice, but not always correctly. Taking the recommendation over 70% of answered the questions correctly. More than 95% of the two surveyed agreed, that they are able to give advice on care. 18.87% of Group I, and 20.97% of Group II attend case discussions weekly. Conclusions: Nurses need to supplement their knowledge about the disease, and technical protocol. Additional information needed to develop the competencies of communication among nurses. I would consider to develop a practice guideline on the disease, which members of the care team will uniformly apply. ]
