[Epidemiology of hypertension in Hungary]

[The most important aspects of Hungarian hypertension epidemiology are demonstrated. The prevalence of hypertension is continuously increasing and in 2011 from the nearly 10 million population exceeded 3 million. Its occurrence rises in parallel with the increase in of systolic blood pressure and the progression of age, and exceeds 60% in those aged over 65 years. The average blood pressure is increasing in children and adolescents, and isolated systolic hypertension is often observed in those over 65-70 years. The situation is closely related to overweight in the young, while in elderly the atherosclerotic process results in low diastolic blood pressure. The prevalence is significantly different in some regions with the highest incidence in South-Transdanubia and North- Hungary. Overall, there is a higher incidence in women. Among known hypertensive patients, the ratio of men is greater till 40-45 years; thereafter, there will be an equilibration and in elderly women the incidence will be higher. It is interesting, that the morbidity and mortality have significantly increased in hypertension and diabetes since 2004, while the incidence of myocardial infarction and stroke has significantly decreased. As concerns organ damage, an increase in occurrence of left ventricular hypertrophy and microalbuminuria has been observed. Metabolic syndrome, diabetes mellitus and ischemic heart disease were the most frequent comorbidities.]

[Guide for using methods of cardiovascular risk estimation]


[Author analyzes the most frequently used, reliable cardiovascular risk estimation methods. Only those methods are useful that meet to the american and european prevention guidelines and were edited by processing of the great databases. The best known risk estimation methods are continuously is expanding by growing body of knowledge and by another iseful risk factors, because the only way in order to successfully prevent the cardiovascular diseases. Two types of method-groups are known: 1. Traditional narrowed form for the general practisers and for the large section of population, 2. Methods with a broader analysis and more risk factors for the specialists and scientists. In Europe PROCAM and Heart Score and their variations are most useful for the practical medical work, but it is possible to use the american ASCVD and Reynolds risk estimation methods as well using the help of the national correction factors. The methods with wider analysis and much more risk factors as the Mayo Clinic mode, Qrisk2 and IHMRS are suitable in assisting of the epidemiologic studies or scientific work.]

[Genetic diagnostics of the trombosis risk]

[The cardiovascular system and the coalugation process play essential role in regulating the homestasis of the human body. Thrombuses may appear in veins (venous thrombosis) as well as arteries (arterial thrombosis) that may cause a wide range of ischemic vascular diseases. By mapping genetic risk factors that may accelarate the development of thrombosis, the quality of medical preventions and therapies can be improved. The most frequent gene mutations (FII, FV, PAI-1, MTHFR and EPCR gene polimorphisms) can be tested by methods based on PCR, real-time PCR and macroarray techniques. Professionals may use genetic results for selecting appropriate and optimal therapies based on the context of a patient’s medical history.]

[Association between cyclothymic affective temperament and hypertension]


[Affective temperaments (cyclothymic, hypertymic, depressive, anxious, irritable) are stable parts of personality and after adolescent only their minor changes are detectable. Their connections with psychopathology is well-described; depressive temperament plays role in major depression, cyclothymic temperament in bipolar II disorder, while hyperthymic temperament in bipolar I disorder. Moreover, scientific data of the last decade suggest, that affective temperaments are also associated with somatic diseases. Cyclothymic temperament is supposed to have the closest connection with hypertension. The prevalence of hypertension is higher parallel with the presence of dominant cyclothymic affective temperament and in this condition the frequency of cardiovascular complications in hypertensive patients was also described to be higher. In chronic hypertensive patients cyclothymic temperament score is positively associated with systolic blood pressure and in women with the earlier development of hypertension. The background of these associations is probably based on the more prevalent presence of common risk factors (smoking, obesity, alcoholism) with more pronounced cyclothymic temperament. The scientific importance of the research of the associations of personality traits including affective temperaments with somatic disorders can help in the identification of higher risk patient subgroups.]

[Nowdays the goal of the antihypertensive treatment is to decrease the cardiovascular risk of the patients by treating the associated diseases and possible hypertensive end-organ damages. Losartan is an antihypertensive drug with all these capabilities and placebo-like side-effect profile. Besides the most important effects of losartan, which are mediated by blocking the angiotensin II. type 1. receptors, it also has other, molecule specific beneficial effects. The most important, well documented pleiotropic effect of losartan is its uricosuric effect. The pleiotropic effects together with the blockade of the angiotensin II. receptors are considered more and more important in the hypertensive end-organ protection and in the treatment of associated diseases in hypertensive patients.]

[Hungarian data about the ramipril based free or fixed doses combination therapy in connection with patients adherence]


[In the treatment of chronic diseases an appropriate therapeutic effect is only achieved with a long-term medication. Hypertension is no exception, because to achieve target blood pressure is of outstanding importance for the prevention of cardiovascular complications. The use of fixed drug combinations according to national and international data increases the patients adherence by 20 percent. In our article we present the one year patients adherence treated with ramipril and amlodipine free combination, ramipril/amlodipine fixed combination and ramipril/hydrochlorothiazide fixed combination in hypertension.]

[Change in the approach of the treatment of hypertension in Hungary. Five years results of the „Live below 140/90!” Program]

[One of the potentials of the effective fight against endemic diseases is their exposition and the recognition of their dangers and risk factors. The other possibility is to increase the professional knowledge of the medical and healthcare employees along with the patients’ co-operation. For the prevention of complications and the adequate treatment of hypertension an extensive compliance program, called „Live below 140/90!” was initiated by the Hungarian Society of Hypertension in 2005. The mission was to give knowledge to the non-professional public about the symptoms of the disease and how to get information about it while helping patients’ relatives. The first message was the “Hit the target blood pressure value!”. With careful planning, treatment and taking of medicines the next phase could begin. The ratio of the patients who reached the target blood pressure increased by 5% during the two years of the Program between 2005 and 2007 therefore the message changed to “Hold the blood pressure there!”. The next step in the Program was to prevent the forming of complications and to treat the co-morbidities effectively among patients with pre-diabetes or diabetes and hypertension in 2008. The slogan was “Prevent the complications!”. As part of the Program we organized a roadshow named the “Day of caring!” and we announced the “Conscious Care” substudy focused on the public summoning about the stroke which is the most dangerous complication of hypertension. The year of 2011 is an absolutely new beginning in the communication of the Program since we started to use some very modern tools of the 21st century including YouTube, Facebook and others for the better education of the people. Based on the results of the initial Program we got to know the risks, co-morbidities, complications and the characteristics of the Hungarian hypertensive patients. We have recognized that most of the patients belong to the high and very high risk hypertensive category. Also more than 30 percent of them have a pre-diabetes condition. We have found that increased caring helps to build up the patients’ co-operation which in return improves the decrease of their blood pressure significantly. The Program therefore continues in 2011! Our intention is to enlarge the Hungarian Hypertension Register database and to get to know more and more epidemiologic and therapeutic features of the hypertension disease.]

[Hemodynamic approach of the treatment of hypertension. The possible role of impedance cardiography in the tailored antihypertensive therapy]


[It is important to achieve the blood pressure targets in the course of antihipertensive therapy. It will be beneficial to increase the proportion of patients on blood pressure target to avoid adverse cardiovascular consequences. In hypertension the main definition the elevation of blood pressure alone, but in hypertension are several abnormalities of cardiac output, systemic vascular resistance, stroke volume and arterial compliance too. Impedance cardiography is a simple highly accurate non-invasive device to assess hemodynamic parameters in hypertension. Hemodynamic assessment may help in the treatment of hypertension. Various authors report the improving rate of blood control rate using impedance cardiography which may decrease the risk of target organ damage.]
