[Aquired non-inflammatory and non-traumatic hypoplasia-dysplasia of the femoral neck Report of two cases]

[Recently DiFazio et al reported four children with remarkable femoral neck deformity who had extracorporeal membrane oxygenation (ECMO) therapy. We report two patients with similar femoral neck changes who did not have ECMO treatment but have had complicated, strenuous first few weeks after birth. The first patient had been operated on twice for diaphragm repair at the age of 18 hours and 1 month. The second patient had cardiac surgery at the age of two months, with one hour of extracorporeal cardiorespiratory bypass. We agree with DiFazio et al and believe that four of their patients and both of ours have femoral neck growth cartilage injury secondary to hypoxia, vascular disturbance and coagulopathy.]

[Pneumoportogram without intestinal pneumatosis]

[INTRODUCTION - Classical radiological signs of ischemic bowel wall necrosis are the presence of gas in the affected bowel wall and intrahepatic gas in the portal venous system. CASE REPORT - A 76-year-old male patient was admitted to the hospital with the suspicion of ileus and perforation. Plain abdominal X-ray showed presence of gas in the portal venous system in addition to small bowel and colonic ileus. Pneumoportogram was present without intestinal pneumatosis. Abdominal laparotomy revealed extensive bowel wall necrosis and no surgical solution was possible. After surgery the patient died. CONCLUSION - Reviewing our case and the medical literature, the significance of plain abdominal X-ray in addition to abdominal CT and ultrasound examination should be emphasized. This case report helps to differentiate the origin of intrahepatic gas in order to select proper therapeutical approach.]

[Effect of two month positive airway pressure therapy on the structure of sleep, cognitive function and anxiety]

[Obstructive sleep apnea is a common disorder, characterized by repeated episodes of upper airway obstruction during sleep, resulting intermittent hypoxia and disruption of the normal sleep pattern, which caused cognitive dysfunction in these patients. Nasal continuous positive airway pressure is the treatment of choice for this disorder. The aim of the study is to evaluate the effect of short-term positive airway pressure on sleep pattern (polisomnographic measures), cognitive function and anxiety. Twenty four newly diagnosed and previously untreated patients with obstructive sleep apnea were evaluated a battery of neuropsychological tests before and after 2 and a half months of the treatment. We focused on working memory, short and long-term episodic memory, executive functions, anxiety and subjective sleepiness. Our results showed that the two and half month of treatment improved the respiration during sleep, sleep pattern and the subjective sleepiness. We found improvement in short- and long-term verbal memory, and complex working memory. Despite of treatment we did not find improvement in visuospatial learning. These results reveal that 2 and a half months of positive airway pressure treatment restored not only the normal respiration during sleep and normal sleep pattern, but also the cognitive functions. Our study suggests that cognitive dysfunction is at least partial reversible in obstructive sleep apnea patients after positive airway pressure treatment.]

[The role of hypoxia in tissue regeneration and in development of amplified aggressive fenotypes in malignant cancer]

[Several diseases are accompanied by hypoxic stress; elimination of it is an important physiological process. Our body provides a protective function which delays damage and destruction by hypoxia. In case of necrosis, it provides the mop up of the damaged area. This security system starts the regeneration in cells of the hypoxic zone which surrounds the necrotic area, resulting in the survival of the cells in hypoxic environment and ensuring the handling of the necrosis. The key molecules of the system are the hypoxia-induced factor molecules. The review discusses the physiological role of tissue hypoxia and normoxia and its effects on tissue regeneration. The physiological system triggered by the hypoxia-induced factor plays an important role in embryonic development, in wound healing and in numerous diseases (eg. myocardial infarction, stroke, vaculities, etc). Unfortunately, this system also plays a key role in several malignant tumors by rising the development of cells with increased aggressive fenotypes as well. The physiological process of regeneration starts in the hypoxic tumor cells aided by the hypoxia-induced factor system. The process results in neovascularization, and in the case of tissue damage, in the mop up of the necrotic tissue and in the restoration of tissue oxygenisation. However, after the formation of the new vascular network, tumor cells accustomed to hypoxia will not die but keep their original uncontrolled proliferation and anaerobic nature. Moreover the malignant nature of the cells will be increased by the genetic changes generated by the system of hypoxia- induced factors. The role of the hypoxia-induced factor system in tumor progression is discussed by the example of one of the most malignant tumors, malignant melanoma.]
